Development of an Integrative Program for Patients with Alcohol Dependence Who were at Risk of Suicide

This research and development aimed to study the situation, develop and evaluate the effectiveness of an integrative program for patients with alcohol dependence who were at risk of suicide. The study was conducted in three phases: (1) situational analysis; (2) program development; and (3) program effectiveness evaluation. The sample in phase 1 consisted of 10 patients with alcohol dependence, 10 caregivers, and 7 professional nurses. The sample in phase 3 composed of 66 patients with alcohol dependence who were at risk of suicide, randomly assigned to 33 patients in the experimental group and 33 patients in the control group. Data collection instruments included: (1) a demographic questionnaire, (2) the Alcohol Use Disorders Identification Test (AUDIT) with a reliability coefficient of 0.86, and (3) the Suicide Assessment Form (8Q) with a reliability coefficient of 0.90. The research intervention was the integrative program for patients with alcohol dependence who were at risk of suicide, yielding a content validity index of 0.93. Data were analyzed using descriptive statistics, the Wilcoxon signed-rank test, and the Mann–Whitney U test.
The study found that the patients with alcohol dependence at risk of suicide had a negative view of their problems, used avoidance and impulsive coping mechanisms, and lacked of motivation to stop drinking. The integrative program consisted of 8 individual sessions: (1) motivation modification; (2) promoting self-care capabilities to prevent relapse; (3) providing education on alcohol consumption and suicide risk; (4) providing education on problem-solving therapy; (5) developing an understanding of problem-solving and problem-solving methods; and (6-8) training on cognitive reframing and problem-solving skills. The effectiveness evaluation of the program revealed that the mean scores of drinking behavior and suicide risk of the experimental group after ending the program were significantly lower than before the program (Z -5.01, and Z -5.02, p < .01, respectively). The mean scores of drinking behaviors and suicide risk of the experimental group after ending the program were significantly lower than the control group (Z -6.87, and Z -4.20, p < .01, respectively).
