บทคัดย่อ
การวิจัยและพัฒนานี้ มีวัตถุประสงค์เพื่อพัฒนาระบบการพยาบาลผู้สูงอายุ โรงพยาบาลนพรัตนราชธานี โดยใช้แนวคิดทฤษฎีระบบ แบ่งเป็น 3 ระยะ คือ (1) วิเคราะห์สถานการณ์การดูแลผู้ป่วยสูงอายุ (2) พัฒนาระบบการพยาบาลผู้ป่วยสูงอายุ และ (3) ประเมินผลลัพธ์การใช้ระบบการพยาบาลฯ กลุ่มตัวอย่าง ประกอบด้วย ผู้ป่วยสูงอายุ จำนวน 390 ราย และพยาบาลวิชาชีพ จำนวน 339 ราย จากหอผู้ป่วยในที่เกี่ยวข้อง เครื่องมือที่ใช้ ได้แก่ ระบบการพยาบาลฯ ที่พัฒนาขึ้น แบบวัดความรู้ การปฏิบัติ ทัศนคติ และความพึงพอใจของผู้ป่วยและพยาบาลวิชาชีพ ตรวจสอบความตรงเชิงเนื้อหาของเครื่องมือ โดยผู้ทรงคุณวุฒิ 3 ท่าน ได้ค่า CVI .88 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา Content analysis และ Paired t-test
ผลการวิจัย พบว่า ระบบการพยาบาลผู้สูงอายุในโรงพยาบาลนพรัตนราชธานี มี 5 องค์ประกอบ ได้แก่ (1) นโยบาย; (2) คณะกรรมการฯ; (3) แนวปฏิบัติ; (4) การพัฒนาสมรรถนะพยาบาลวิชาชีพ และ (5) องค์ความรู้ด้านการปรึกษา การบันทึก การรายงาน และการนิเทศติดตาม ผลลัพธ์การใช้ระบบการพยาบาลฯ พบว่า สมรรถนะด้านความรู้และการปฏิบัติของพยาบาลดีขึ้นอย่างมีนัยสำคัญทางสถิติ (p < .001) ร้อยละของการเกิดพลัดตกหกล้ม การเกิดแผลกดทับ และการติดเชื้อปอดอักเสบ ลดลงอย่างมีนัยสำคัญทางสถิติ (p < .001) ผู้ป่วยและพยาบาลวิชาชีพมีความพึงพอใจในระดับสูง มากกว่าร้อยละ 90 สรุปการศึกษาวิจัยครั้งนี้แสดงให้เห็นว่าการพัฒนาระบบการพยาบาลที่เหมาะสมเป็นสิ่งสำคัญต่อการป้องกันภาวะแทรกซ้อน และช่วยยกระดับคุณภาพชีวิตของผู้สูงอายุ
