บทคัดย่อ
งานวิจัยและพัฒนาครั้งนี้มีวัตถุประสงค์เพื่อพัฒนาและศึกษาผลลัพธ์การใช้แนวปฏิบัติการพยาบาลผู้ป่วยสูงอายุ ที่ทำผ่าตัดใส่ข้อสะโพกเทียมโดยใช้หลักฐานเชิงประจักษ์ ดำเนินการ 4 ขั้นตอน คือ (1) การค้นหาปัญหาทางคลินิก (2) การสืบค้นหลักฐานเชิงประจักษ์ (3) การพัฒนาและนำแนวปฏิบัติการพยาบาลไปทดลองใช้ (4) การนำแนวปฏิบัติการพยาบาลที่ปรับปรุงแล้วไปใช้จริง กลุ่มตัวอย่างเลือกแบบเจาะจง คือ ผู้ป่วยสูงอายุที่ได้รับการผ่าตัดใส่ข้อสะโพกเทียม จำนวน 26 คน และพยาบาลวิชาชีพผู้ใช้แนวปฏิบัติการพยาบาล จำนวน 18 คน เครื่องมือดำเนินการวิจัย คือ (1) แนวปฏิบัติการพยาบาล (2) คู่มือการปฏิบัติ ค่า AGREE II ร้อยละ 88.70 เครื่องมือที่ใช้ในการรวบรวมข้อมูลวิจัย ได้แก่ (1) แบบสอบถามความพึงพอใจของพยาบาลต่อการใช้แนวปฏิบัติการพยาบาล มีค่าความเชื่อมั่น 0.94 (2) แบบประเมินความสามารถปฏิบัติกิจวัตรประจำวัน (3) แบบบันทึกข้อมูลผลลัพธ์ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และสถิติ แมน -วิทนีย์
ผลการวิจัย พบว่า แนวปฏิบัติการพยาบาลผู้ป่วยสูงอายุที่ทำผ่าตัดใส่ข้อสะโพกเทียมโดยใช้หลักฐาน เชิงประจักษ์ มี 4 หมวด คือ ระยะก่อนผ่าตัด ระยะหลังผ่าตัด ระยะฟื้นฟูสภาพก่อนจำหน่าย และการดูแลต่อเนื่อง หลังใช้แนวปฏิบัติพยาบาลวิชาชีพมีความพึงพอใจอยู่ในระดับมาก มีค่าเฉลี่ยเท่ากับ 3.66 (SD .59) และผู้ป่วยสูงอายุที่ทำผ่าตัดใส่ข้อสะโพกเทียม มีความสามารถปฏิบัติกิจวัตรประจำวันเพิ่มขึ้น ระยะเวลารอคอยผ่าตัด การเกิดภาวะแทรกซ้อนหลังผ่าตัด ค่าใช้จ่ายและจำนวนวันนอนเฉลี่ยลดลง อย่างมีนัยสำคัญทางสถิติ (p < 0.05)
