Abstract


This research and development study aimed to develop and evaluate outcomes of the clinical nursing practice guideline (CNPG) for the elderly patients undergone total hip arthroplasty using evidence-based practice. The study consisted of 4 phases: (1) evidence triggered phase, (2) evidence supported phase, (3) evidence observed phase, and (4) evidence-based phase. The purposive sample selection included 26 elderly patients undergone hip arthroplasty and 18 professional nurses who use the clinical practice guidelines. The research instrument consisted of (1) the nursing practice guideline, (2) the practical manual yielding AGREE II of 88.70 per cent.  The data collection tools composed of (1) a satisfaction questionnaire of registered nurses toward CNPG using, (2) Barthel Index, and (3) the clinical outcomes record form. Data were analyzed using descriptive statistics and Mann-Whitney U test.


The results revealed that the guideline for elderly patients undergone total hip arthroplasty using evidence-based practice consisted of 4 phases of care: (1) preoperative phase, (2) postoperative phase, (3) rehabilitation before discharge, and (4) continuous care. After the CNPG implementation, the results revealed that the satisfaction score of professional nurses was at a high level (Mean 3.66, SD .59). The elderly patient with total hip arthroplasty had increased the ability to perform daily activities; while the waiting time for operation, post operation complication, cost, and length of stay were decreased yielding statistically significant level (p < 0.05).
