บทคัดย่อ
การวิจัยและพัฒนานี้ มีวัตถุประสงค์เพื่อสร้างรูปแบบพัฒนาความรู้ความสามารถของพยาบาลวิชาชีพเรื่องการเตรียมผู้ป่วยก่อนผ่าตัดเปิดช่องท้องอย่างปลอดภัยจากภาวะแทรกซ้อนแบบออนไลน์ โดยใช้แนวคิดทฤษฎีการสร้างความรู้ด้วยตนเอง (constructivism) และกระบวนการสนทนากลุ่ม กลุ่มตัวอย่าง ได้แก่ พยาบาลวิชาชีพโรงพยาบาลประจวบคีรีขันธ์ที่ปฏิบัติงานดูแลผู้ป่วยผ่าตัดเปิดช่องท้อง การวิจัยแบ่งเป็น 4 ระยะ (1) วิเคราะห์ข้อมูลพื้นฐาน (2) การสร้างรูปแบบ (3) การทดลองใช้รูปแบบ และ (4) การประเมินประสิทธิผลและรับรองรูปแบบ เครื่องมือที่ใช้ คือ รูปแบบพัฒนาความรู้ความสามารถของพยาบาลวิชาชีพเรื่อง การเตรียมผู้ป่วยก่อนผ่าตัดเปิดช่องท้องอย่างปลอดภัยจากภาวะแทรกซ้อนแบบออนไลน์ แบบวัดความรู้ความสามารถของพยาบาลวิชาชีพ และแบบสอบถามความพึงพอใจของพยาบาลวิชาชีพ วิเคราะห์ข้อมูลด้วยความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน independent t-test และ paired t-test
ผลการวิจัย พบว่า (1) รูปแบบพัฒนาความรู้ความสามารถของพยาบาลวิชาชีพฯ ประกอบด้วย ขั้นตอนการผ่าตัดเปิดช่องท้อง การเตรียมความพร้อมผู้ป่วยก่อนผ่าตัดเปิดช่องท้อง และคำแนะนำในการเตรียมความพร้อมก่อนการผ่าตัดเปิดช่องท้อง (2) ผลการนำรูปแบบฯ ไปใช้ พบว่า พยาบาลวิชาชีพกลุ่มทดลองมีค่าเฉลี่ยระดับความรู้ความสามารถสูงกว่าก่อนใช้รูปแบบ และสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p 0.00) พยาบาลผู้ใช้มีความพึงพอใจต่อรูปแบบ โดยรวมในระดับมาก (Mean 4.35, SD 0.59) และ (3) ภายหลังการใช้รูปแบบ จำนวนผู้ป่วยที่ถูกเลื่อนหรืองดผ่าตัดจากสาเหตุที่ควบคุมได้ลดลง และไม่พบภาวะแทรกซ้อนต่อระบบหัวใจและหลอดเลือด
