บทคัดย่อ
การวิจัยและพัฒนานี้ มีวัตถุประสงค์เพื่อพัฒนาและศึกษาผลของแนวปฏิบัติพยาบาลทางคลินิกสำหรับวิสัญญีพยาบาลในการดูแลผู้ป่วยที่ได้รับการผ่าตัดเปลี่ยนลิ้นหัวใจไมตรัลภายใต้การระงับความรู้สึกแบบทั่วไป มี 4 ขั้นตอน (1) ศึกษาสถานการณ์ (2) พัฒนาแนวปฏิบัติฯ (3) นำแนวปฏิบัติฯไปใช้ และ (4) ประเมินผลแนวปฏิบัติฯ กลุ่มตัวอย่าง ได้แก่ (1) วิสัญญีพยาบาล 55 ราย และ (2) ผู้ป่วยที่ได้รับการผ่าตัดเปลี่ยนลิ้นหัวใจไมตรัล 30 ราย เครื่องมือวิจัย คือ แนวปฏิบัติฯ ตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 3 ท่าน ค่าCVI เท่ากับ 1 เครื่องมือเก็บข้อมูล ได้แก่ (1) แบบสอบถามความความคิดเห็นต่อแนวปฏิบัติฯ (2) แบบประเมินการปฏิบัติตามแนวปฏิบัติฯ (3) แบบทดสอบความรู้ในการให้ยาระงับความรู้สึกฯ (4) แบบสอบถามความพึงพอใจของผู้ใช้บริการ (5) แบบประเมินความสามารถในการดูแลตนเอง ค่าสัมประสิทธิ์แอลฟาของครอนบาค เท่ากับ .87, .81, .79, .82 และ .82 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และ t-test
ผลการศึกษา พบว่า แนวปฏิบัติฯ ประกอบด้วย 3 แนวปฏิบัติพยาบาล คือ ก่อน ระหว่าง และหลังให้ยาระงับความรู้สึก ผลการใช้แนวปฏิบัติฯ พบว่า วิสัญญีพยาบาล มีคะแนนความรู้สูงกว่าก่อนการใช้แนวปฏิบัติฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 มีความคิดเห็นโดยรวมอยู่ในระดับมากที่สุด (Mean 3.61, SD .45) และมีคะแนนการปฏิบัติโดยรวมอยู่ในระดับมาก (Mean 3.46, SD .48) ผู้ใช้บริการมีความพึงพอใจต่อแนวปฏิบัติพยาบาลฯ โดยรวมอยู่ในระดับมากที่สุด (Mean 3.56, SD .50) ผลลัพธ์ด้านคุณภาพบริการพยาบาล พบว่า อัตราผู้ป่วยได้รับการเยี่ยมประเมินก่อนการระงับความรู้สึกและหลังการระงับความรู้สึก ร้อยละ 100 และไม่เกิดอุบัติการณ์ภาวะแทรกซ้อนระหว่างการระงับความรู้สึก
