ประสิทธิผลของโปรแกรมลดการบริโภคโซเดียมเปรียบเทียบกับการดูแลมาตรฐานเพื่อชะลอการเสื่อมของไตในผู้ป่วยเบาหวานที่มีไตวายเรื้อรังระยะที่ 3

บทคัดย่อ


                        การวิจัยเป็นแบบกึ่งทดลองครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมลดการบริโภคโซเดียมรายบุคคล ในผู้ป่วยเบาหวานที่มีไตวายเรื้อรังระยะที่ 3 คัดเลือกกลุ่มตัวอย่างแบบสุ่มเฉพาะจงเจาะตามคุณสมบัติที่กำหนด 128 คน แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 64 คน ติดตามผลการใช้โปรแกรมเป็นระยะเวลา 6 เดือน เครื่องมือที่ใช้ดำเนินการวิจัย คือ 1) โปรแกรมลดการบริโภคโซเดียม ประกอบด้วย สื่อองค์ความรู้และเครื่องมือลดเค็มลดโซเดียม ตรวจสอบคุณภาพของโปรแกรมลดการบริโภคโซเดียมด้วยผู้ทรงคุณวุฒิ 4 คน ค่า CVI 0.90 และค่าความเชื่อมั่น 0.90  2) เครื่องมือที่ใช้เก็บรวบรวมข้อมูลได้แก่ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามการบริโภคอาหารของผู้ป่วยเบาหวานที่มีไตวายเรื้อรัง ตรวจสอบคุณภาพโดยผู้ทรงคุณวุฒิ ค่า CVI 0.90 และค่าความเชื่อมั่น 0.90 วัดผลลัพธ์ทางคลินิกด้วยค่าอัตราการกรองของไต ค่าปริมาณโซเดียมในปัสสาวะ ค่าความเข้มข้นของเลือด ค่าระดับน้ำตาลในเลือด ค่าระดับความดันโลหิต วิเคราะห์ข้อมูลด้วย ความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติถดถอยแบบ Mixed-effect


                        ผลการวิจัย พบว่าผู้ป่วยเบาหวานที่มีภาวะไตวายเรื้อรังระยะที่ 3 ในกลุ่มทดลองมีค่าระดับ eGFR, Urine Sodium, ค่าระดับความดันโลหิต ทั้ง SBP และ DBP ต่ำกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติที่ระดับ <.05 จึงควรนำโปรแกรมนี้ไปใช้กับผู้ป่วยเบาหวานที่มีไตวายเรื้อรังระยะที่ 3 เพื่อช่วยลดค่าระดับความดันโลหิตและชะลอไตเสื่อมได้ ข้อเสนอแนะในการนำผลการวิจัยไปใช้ ผู้ป่วยเบาหวานที่มีภาวะแทรกซ้อนทางไตระยะที่ 3 ควรมีโอกาสเข้าร่วมโปรแกรมนี้เพื่อปรับเปลี่ยนพฤติกรรมสุขภาพเกี่ยวกับอาหารลดโซเดียม
