บทคัดย่อ
โครงการนี้ มีวัตถุประสงค์ เพื่อพัฒนาระบบการจัดบริการพยาบาลในคลินิกหมอครอบครัว และพัฒนาศักยภาพของพยาบาลที่ปฏิบัติงานในคลินิกหมอครอบครัว ดำเนินการ 2 ระยะ คือ พัฒนาระบบการจัดบริการพยาบาลในคลินิกหมอครอบครัว และ นำระบบการจัดบริการพยาบาลในคลินิกหมอครอบครัวสู่การปฏิบัติ คัดเลือกกลุ่มเป้าหมายแบบเจาะจงเป็นพยาบาลที่ปฏิบัติงานในคลินิกหมอครอบครัว 442 คน และครอบครัวที่รับผิดชอบ 442 ครอบครัว เครื่องมือประเมินผลโครงการ คือ 1) แบบประเมินความรู้ของพยาบาล 2) แบบประเมินการปฏิบัติตามระบบการจัดบริการพยาบาลในคลินิกหมอครอบครัว 3) แบบประเมินความพึงพอใจของพยาบาล 4) แบบประเมินความสามารถของครอบครัวดูแลสุขภาพตนเองที่บ้าน 5) แบบบันทึกการเข้าถึงบริการ วิเคราะห์ข้อมูลด้วยร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตราฐาน และสถิติ Paired t – test
ผลการดำเนินการระยะที่ 1 พบ 3 องค์ประกอบของระบบการจัดบริการพยาบาลในคลินิกหมอครอบครัว คือ 1) ปัจจัยนำเข้า 5 ด้าน: การออกแบบบริการ บุคลากรทางการพยาบาล การเงินและอุปกรณ์เครื่องมือเครื่องใช้ ระบบข้อมูลข่าวสารสุขภาพ และภาวะผู้นำและธรรมมาภิบาล 2) มีการจัดบริการ 5 รูปแบบ: การจัดบริการในสถานบริการ การจัดบริการเพื่อการดูแลต่อเนื่อง การจัดบริการเชิงรุกในชุมชน การสร้างเครือข่ายและการมีส่วนร่วม และการให้ข้อมูลและคำปรึกษา 3) ผลลัพธ์ บรรลุเป้าหมาย คือ ครอบครัวสามารถดูแลตนเองที่บ้าน และประชาชนเข้าถึงบริการ ผลการดำเนินงานระยะที่ 2 พบ พยาบาลมีความรู้สูงขึ้นอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ปฏิบัติตามระบบได้มาก (Mean 4.13, SD .48) และพึงพอใจต่อระบบในระดับมาก (Mean 4.12, SD .78) ผู้ป่วยและครอบครัวสามารถจัดการดูแลสุขภาพตนเองที่บ้าน ได้มาก (Mean 3.82, SD .49) และประชาชนเข้าถึงบริการได้มากขึ้น ร้อยละ 87.65
