การพัฒนาแนวปฏิบัติทางการพยาบาลสำหรับป้องกันการเกิดแผลกดทับในโรงพยาบาลตราด

บทคัดย่อ

                     การวิจัยนี้เป็นการวิจัยและพัฒนามีวัตถุประสงค์เพื่อพัฒนาและศึกษาผลของแนวปฏิบัติทางการพยาบาลสำหรับป้องกันการเกิดแผลกดทับ ประกอบด้วยการศึกษาสถานการณ์ การพัฒนา การทดลองใช้และการประเมินผลลัพธ์การใช้แนวปฏิบัติทางการพยาบาล กลุ่มตัวอย่างเลือกแบบเจาะจง ได้แก่พยาบาลวิชาชีพ จำนวน 90 คน  และ ผู้ป่วยกลุ่มเสี่ยงสูง  จำนวน 100  คน  เครื่องมือที่ใช้ในการดำเนินการวิจัย คือ แนวปฏิบัติทางการพยาบาลสำหรับป้องกันการเกิดแผลกดทับ ตรวจสอบด้วยผู้เชี่ยวชาญ 5 คน ได้ค่าความเชื่อมั่น 1.0  เก็บรวบรวมข้อมูล ด้วย 1) แบบประเมินความเสี่ยงต่อการเกิดแผลกดทับของบราเดน   2) แบบวัดความรู้เรื่องการป้องกันการเกิดแผลกดทับ  3)  แบบประเมินการปฏิบัติตามแนวปฏิบัติทางการพยาบาล 4) แบบสอบถามความพึงพอใจหลังการใช้แนวปฏิบัติทางการพยาบาล  วิเคราะห์ข้อมูลโดยใช้ความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และ  t-test 


                      ผลการวิจัย พบว่า แนวปฏิบัติทางการพยาบาลสำหรับป้องกันการเกิดแผลกดทับ มี 6 หมวด: 1) การประเมินความเสี่ยงการเกิดแผลกดทับ 2) การป้องกันการเกิดแรงกดและเสียดสี 3) การดูแลภาวะโภชนาการ  4) การประเมินผิวหนัง 5) การให้ความรู้แก่เจ้าหน้าที่และญาติ 6) การจัดการสิ่งแวดล้อม ผลลัพธ์ ด้านผู้ให้บริการ พบว่า ความรู้เรื่องป้องกันการเกิดแผลกดทับก่อนและหลังการอบรม แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (P<.05) การปฏิบัติตามแนวปฏิบัติทางการพยาบาล สูงกว่าเกณฑ์เป้าหมาย และแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (P<.05) ความพึงพอใจของพยาบาลหลังใช้แนวปฏิบัติ สูงกว่าเกณฑ์เป้าหมาย  และแตกต่างกันอย่างมีนัยสำคัญทางสถิติ  (P<.05) ผลลัพธ์ด้านผู้ใช้บริการและด้านคุณภาพการพยาบาล ไม่พบอุบัติการณ์การเกิดแผลกดทับภายหลังใช้แนวปฏิบัติทางการพยาบาล  สรุปได้ว่าแนวปฏิบัติทางการพยาบาลสำหรับป้องกันการเกิดแผลกดทับ สามารถพัฒนาความรู้ ความสามารถของพยาบาลวิชาชีพในการป้องกันการเกิดแผลกดทับในผู้ป่วยกลุ่มเสี่ยงได้อย่างมีประสิทธิผลและเพิ่มคุณภาพการพยาบาลได้
