Recently Graduated Nurses: A New Clinical Supervision Model Developed at Nongkhai Hospital

Abstract

This research and development study aimed to develop a clinical supervision model, based on Proctor's model, for recently graduated professional nurses at Nong Khai Hospital. Model was    developed as a process of clinical supervision with a manual and video clip on clinical supervision for newly graduated professional nurses. The impact of developed clinical supervision model was     evaluated on: 1) clinical supervision skills of nursing supervisors, 2) nursing supervisors' satisfaction with the model, 3) competency of recently graduated professional nurses, as well as 4) recently graduated professional nurses’ satisfaction with the developed clinical supervision model. The study conducted 3 phases: 1) needs assessment, 2) design and development, and 3) tryout and implementation. Key informants included 14 head nurses and sub-head nurses, as well as 10 newly graduated professional nurses, all being employees of Nong Khai Hospital. Sample included 11 head nurses and sub-head nurses, as well as 12 newly graduated professional nurses. The research tools were: 1) focus groups and brainstorming question guidelines, 2) a clinical supervision model for newly graduated professional nurses, 3) a questionnaire for both nursing supervisors and newly graduated professional nurses, including personal data, clinical supervisory skills assessment, and satisfaction with the developed supervision model. Content validity indexes for the nursing supervisors' questionnaire were 0.90 and 0.97, respectively, while the Cronbach's alpha coefficients were 0.89 and 0.85, respectively. Content validity indexes for the newly graduated professional nurses' questionnaire were 0.90 and 0.96, respectively, while the Cronbach's alpha coefficients were 0.88 and 0.85, respectively. Data were analyzed using frequency, percentage, mean, standard deviation, as well as the Wilcoxon signed-rank test. The results were as follows.

The clinical supervisory skills of nursing supervisors as well as competency of newly graduated professional nurses were significantly higher than before using the developed model  (p<.01).   Satisfaction with the developed supervision model of nursing supervisors as well as satisfaction with the developed supervision model of newly graduated professional nurses were at a high level.

Therefore, this model, based on Procter's concept, can enhance clinical supervision skills, competencies and satisfaction.
