การถ่ายโอนภารกิจโรงพยาบาลส่งเสริมสุขภาพตำบลสู่องค์กรปกครองส่วนท้องถิ่น: การทบทวนวรรณกรรมอย่างเป็นระบบ

การถ่ายโอนภารกิจโรงพยาบาลส่งเสริมสุขภาพตำบลสู่องค์กรปกครองส่วนท้องถิ่นเป็นนโยบายสำคัญของการกระจายอำนาจไทย แต่การดำเนินการประสบปัญหาความซับซ้อนและความท้าทายหลายประการ การศึกษานี้มีวัตถุประสงค์เพื่อทบทวนและสังเคราะห์หลักฐานเชิงประจักษ์เกี่ยวกับความพร้อมขององค์กรปกครองส่วนท้องถิ่นในการรับโอนภารกิจโรงพยาบาลส่งเสริมสุขภาพตำบลอย่างเป็นระบบ โดยใช้วิธีการทบทวนวรรณกรรมอย่างเป็นระบบตามแนวทาง PRISMA 2020 Statement ค้นหาข้อมูลจากฐานข้อมูลสากล (PubMed/MEDLINE, Scopus, Web of Science, Cochrane Library) และฐานข้อมูลภาษาไทย (Thai jo, ThaiLIS, TDC) สำหรับงานวิจัยที่ตีพิมพ์ระหว่าง พ.ศ. 2556-2567 ใช้กรอบ PICOS Framework ในการกำหนดเกณฑ์คัดเลือก ประเมินคุณภาพการศึกษาด้วยเครื่องมือมาตรฐาน และสังเคราะห์ข้อมูลด้วยวิธี Narrative synthesis
ผลการศึกษา จากการค้นหาเบื้องต้น 856 รายการ คัดเลือกได้ 42 การศึกษา (งานวิจัยในประเทศไทย 36 เรื่อง และต่างประเทศ 6 เรื่อง) องค์กรปกครองส่วนท้องถิ่นส่วนใหญ่มีความพร้อมในระดับปานกลาง
ร้อยละ 66.70 โดยพบปัญหาหลักในด้านบุคลากรที่ขาดความรู้เฉพาะทาง ร้อยละ 76.20 งบประมาณ
ไม่เพียงพอ ร้อยละ 70.00 และโครงสร้างองค์กรที่ไม่เหมาะสม ร้อยละ 83.30 ปัจจัยแห่งความสำเร็จประกอบด้วย ภาวะผู้นำที่เข้มแข็ง การมีส่วนร่วมของภาคีเครือข่าย การสนับสนุนจากส่วนกลาง การเตรียมความพร้อมอย่างเป็นระบบ กลไกการกำกับดูแลที่ชัดเจน และการจัดการความรู้ที่ต่อเนื่อง

การเตรียมความพร้อมอย่างเป็นระบบ การสนับสนุนจากส่วนกลางอย่างต่อเนื่อง และการสร้างกลไกการกำกับดูแลที่มีประสิทธิภาพ เป็นสิ่งจำเป็นสำหรับการดำเนินนโยบายที่ประสบความสำเร็จ
