Transfer of Health Promoting Hospital Functions to Local Administrative Organizations: A Systematic Literature Review

The transfer of Sub-district Health Promoting Hospital (SHPH) functions to Local Administrative Organizations (LAOs) is a key policy of Thailand's decentralization strategy. However, the implementation faces complex challenges and difficulties. This study aimed to systematically review and synthesize empirical evidence on LAOs readiness for receiving SHPH function transfers. This systematic literature review followed PRISMA 2020 Statement guidelines, searching international databases (PubMed/MEDLINE, Scopus, Web of Science, Cochrane Library) and Thai databases (Thai jo, ThaiLIS, TDC) for the studies published between 2013-2024. PICOS Framework was used to define selection criteria. Study quality was assessed using standard tools, and data synthesis was conducted through narrative synthesis methods.
The results revealed that from 856 initial records, 42 studies were selected (36 Thai studies and 6 international studies). Most LAOs demonstrated moderate readiness levels 66.70%, with major challenges in lack of personnel with specialized expertise 76.20%, insufficient budget allocation 70.00%, and inappropriate organizational structures 83.30%. Success factors included strong leadership, stakeholder network participation, central government support, systematic preparation, clear governance mechanisms, and continuous knowledge management.
LAOs readiness serves as the primary determinant of transfer success. Systematic preparation, continuous central support, and establishing effective governance mechanisms are essential for successful policy implementation.
