การบูรณาการสู่ความยั่งยืนด้านสุขภาพในยุคดิจิทัล: การวิเคราะห์อภิมาน ระบบบริการปฐมภูมิหญิงตั้งครรภ์วัยรุ่น

บทคัดย่อ
การวิจัยนี้เป็นการวิเคราะห์อภิมานแบบผสมผสาน มีวัตถุประสงค์เพื่อ 1) วิเคราะห์ประสิทธิผลของการ บูรณาการเทคโนโลยีดิจิทัลในระบบบริการปฐมภูมิสำหรับหญิงตั้งครรภ์วัยรุ่น 2) ศึกษาปัจจัยที่ส่งผลต่อความยั่งยืน และ 3) พัฒนาข้อเสนอแนะเชิงนโยบาย กลุ่มตัวอย่างคืองานวิจัยจำนวน 38 เรื่อง ที่ตีพิมพ์ระหว่างปี พ.ศ. 2556-2567 ซึ่งผ่านเกณฑ์การคัดเลือกตามคุณสมบัติที่กำหนด เครื่องมือที่ใช้ประกอบด้วยแบบบันทึกการคัดเลือกงานวิจัย แบบประเมินคุณภาพงานวิจัย (RoB 2 และ ROBINS-I) และแบบสกัดข้อมูล การวิเคราะห์ข้อมูลใช้การคำนวณขนาดอิทธิพลรวมโดยใช้โมเดลอิทธิพลสุ่ม วิเคราะห์ความไม่เป็นเอกพันธ์ การวิเคราะห์กลุ่มย่อย การวิเคราะห์การถดถอยเมตา การประเมินอคติในการตีพิมพ์ และการวิเคราะห์เนื้อหาแบบอุปนัย
ผลการศึกษาพบว่า การบูรณาการเทคโนโลยีดิจิทัลมีประสิทธิผลในการเพิ่มการใช้บริการฝากครรภ์ (RR = 1.72, 95% CI: 1.35, 2.18) ปรับปรุงความรู้และพฤติกรรมสุขภาพ และส่งผลดีต่อผลลัพธ์ด้านสุขภาพ แอปพลิเคชันมือถือมีประสิทธิผลสูงสุด (SMD = .64) การบูรณาการเข้ากับระบบที่มีอยู่และการมีส่วนร่วมของผู้มีส่วนได้ส่วนเสียมีความสัมพันธ์กับประสิทธิผลอย่างมีนัยสำคัญ (p <.05) อย่างไรก็ตามประสิทธิผลลดลงเมื่อติดตามผลนานขึ้น และมีความแตกต่างตามระดับรายได้ของประเทศ ปัจจัยที่ส่งผลต่อความยั่งยืนประกอบด้วยปัจจัยหลายระดับ ตั้งแต่ระดับบุคคลถึงระดับนโยบาย
ข้อเสนอแนะสำคัญคือควรส่งเสริมการพัฒนาแอปพลิเคชันมือถือ บูรณาการเข้ากับระบบที่มีอยู่ ส่งเสริมการมีส่วนร่วมของผู้มีส่วนได้ส่วนเสีย และมีการสนับสนุนเชิงนโยบายและงบประมาณอย่างต่อเนื่องเพื่อความยั่งยืนในระยะยาว
