ประสบการณ์ของพยาบาลในการดูแลผู้ป่วยจิตเวชที่มีภาวะปอดอักเสบจากการสำลัก: การวิจัยปรากฏการณ์วิทยาเชิงพรรณนา

บทคัดย่อ
การวิจัยปรากฏการณ์วิทยาเชิงพรรณนานี้ มีวัตถุประสงค์เพื่อบรรยายประสบการณ์ของพยาบาลในการดูแลผู้ป่วยจิตเวชที่มีภาวะปอดอักเสบจากการสำลัก ผู้ให้ข้อมูลเป็นพยาบาลวิชาชีพ หอผู้ป่วยราชทัณฑ์ปันสุข โรงพยาบาลนครปฐม จำนวน 12 ราย เก็บรวมรวมข้อมูลโดยการสัมภาษณ์แบบเจาะลึกร่วมกับการสังเกตพฤติกรรมและบันทึกภาคสนาม และวิเคราะห์ข้อมูลตามแนวทางของโคไลซี
ผลการศึกษาพบว่าประสบการณ์ของพยาบาลในการดูแลผู้ป่วยจิตเวช ได้แก่ 1) การเฝ้าระวังด้วยสายตาแทนเสียงของผู้ป่วย 2) การบูรณาการมาตรการป้องกันการสำลักภายใต้ข้อจำกัดทางจิต 3) การรับมือกับปัญหาที่ต้องเจอภายใต้ความย้อนแย้งของการรักษา และ 4) การพัฒนาศักยภาพพยาบาลเพื่อยกระดับคุณภาพและความปลอดภัยในการดูแลผู้ป่วยจิตเวช และประสบการณ์ที่ค้นพบ คือ การเป็นผู้พิทักษ์ความปลอดภัยของผู้ป่วยภายใต้ความไม่แน่นอน โดยพยาบาลต้องอาศัยทั้งความรู้ ประสบการณ์ การรับรู้เชิงลึก ความไวต่อสัญญาณต่าง ๆ และการตัดสินใจเชิงจริยธรรม เพื่อคงไว้ซึ่งความปลอดภัยและศักดิ์ศรีความเป็นมนุษย์ของผู้ป่วยจิตเวชที่มีความเปราะบางทั้งทางกายและจิตใจ
ผลการวิจัยนี้ สามารถนำไปเป็นข้อมูลพื้นฐานในการจัดทำแนวปฏิบัติที่เฉพาะเจาะจงสำหรับผู้ป่วยที่ได้รับยาจิตเวชกลุ่มเสี่ยงสูง รวมถึงสำหรับผู้ป่วยที่ไม่ให้ความร่วมมือ ที่อาจช่วยลดอุบัติการณ์การสำลักที่นำไปสู่การเกิดปอดอักเสบได้อย่างเหมาะสม รวมทั้ง การนำองค์ความรู้ไปบูรณาการในการเรียนการสอนเพื่อฝึกทักษะการสื่อสารเชิงกลยุทธ์ การดูแลทางกายและทางจิตของนักศึกษาพยาบาลได้
