บทคัดย่อ
การวิจัยเชิงคุณภาพแบบพรรณนานี้ มีวัตถุประสงค์เพื่อศึกษาเสียงสะท้อนของภาคีเครือข่ายต่อปัญหาการดูแลผู้ป่วยจิตเวช และเสนอแนวทางพัฒนาระบบสนับสนุนในชุมชน ผู้ให้ข้อมูลหลักเป็นภาคีเครือข่ายที่เกี่ยวข้องกับการดูแลผู้ป่วยจิตเวชในชุมชนตำบลเกาะแก้ว อำเภอเสลภูมิ จังหวัดร้อยเอ็ด จำนวน 24 คน คัดเลือกแบบเฉพาะเจาะจง เก็บรวบรวมข้อมูลด้วยการสนทนากลุ่ม โดยใช้แนวคำถามสัมภาษณ์กึ่งโครงสร้าง ซึ่งผ่านการตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 3 ท่าน มีค่าดัชนีความสอดคล้อง ระหว่าง .67–1.00 วิเคราะห์ข้อมูลด้วยการวิเคราะห์แก่นสาร
ผลการวิจัยพบ 2 ประเด็นหลัก ได้แก่ 1) ข้อจำกัดเชิงระบบ เช่น การตีตราทางสังคม ความไม่ชัดเจนของบทบาทเครือข่าย การขาดกลไกส่งต่อและติดตามหลังจำหน่าย และการขาดศักยภาพของผู้นำชุมชนและอาสาสมัคร และ 2) แนวทางพัฒนาระบบสนับสนุน ได้แก่ การจัดให้มีทีมผู้เชี่ยวชาญด้านสุขภาพจิตในระดับพื้นที่ การพัฒนาศักยภาพเครือข่ายอย่างต่อเนื่อง การกำหนดระบบดูแลต่อเนื่องหลังจำหน่ายที่มีผู้รับผิดชอบชัดเจน การสนับสนุนงบประมาณจากองค์กรปกครองส่วนท้องถิ่น และการใช้ทุนทางสังคม วัฒนธรรม และศาสนา เป็นกลไกสร้างการยอมรับในชุมชน
โรงพยาบาลส่งเสริมสุขภาพตำบล อาสาสมัครสาธารณสุข และองค์กรปกครองส่วนท้องถิ่น นำผลการวิจัยไปใช้ในการพัฒนาระบบดูแลต่อเนื่องสำหรับผู้ป่วยจิตเวชระดับตำบล โดยกำหนดบทบาทผู้รับผิดชอบของแต่ละหน่วยงานให้ชัดเจน และจัดทำแนวปฏิบัติร่วมกันในการติดตามอาการ เยี่ยมบ้าน และสื่อสารข้อมูลระหว่างหน่วยงานอย่างเป็นระบบ เพื่อให้ผู้ป่วยได้รับการดูแลต่อเนื่องและลดการกลับเป็นซ้ำ
