ผลของรูปแบบการบำบัดการพยาบาลเพื่อป้องกันภาวะสับสนเฉียบพลันในผู้ป่วยสูงอายุที่รับการรักษาในหอผู้ป่วยหนักอายุรกรรม

บทคัดย่อ
การวิจัยแบบกึ่งทดลองนี้มีวัตถุประสงค์เพื่อศึกษาเปรียบเทียบคะแนนภาวะสับสนเฉียบพลันระหว่างกลุ่มทดลองที่ได้รับการดูแลโดยใช้รูปแบบการบำบัดการพยาบาลเพื่อป้องกันภาวะสับสนเฉียบพลันและกลุ่มควบคุมที่ใช้การพยาบาลตามปกติ กลุ่มตัวอย่างที่ใช้ในการวิจัย ได้แก่ ผู้ป่วยสูงอายุกลุ่มเสี่ยงต่อการเกิดภาวะสับสนเฉียบพลัน จำนวน 102 คน แบ่งเป็นกลุ่มควบคุม 51 คน และกลุ่มทดลอง 51 คน คัดเลือกตามเกณฑ์คุณสมบัติที่กำหนดแบบเจาะจง กลุ่มควบคุมได้รับการพยาบาลตามปกติ กลุ่มทดลองได้รับการดูแลโดยใช้รูปแบบการบำบัดการพยาบาลเพื่อป้องกันภาวะสับสนเฉียบพลัน เครื่องมือที่ใช้ในการวิจัย ประกอบด้วย 1) แบบบันทึกข้อมูลส่วนบุคคลของผู้ป่วย 2) แบบประเมินความกระสับกระส่าย-การสงบประสาทของริชมอนด์ 3) แบบประเมินภาวะสับสนเฉียบพลัน 4) เครื่องมือที่ใช้ในการทดลอง ได้แก่ รูปแบบการบำบัดการพยาบาลเพื่อป้องกันภาวะสับสนเฉียบพลันในผู้ป่วยที่รับการรักษาในหอผู้ป่วยวิกฤตอายุรกรรมที่ผู้วิจัยพัฒนาขึ้น ตามรูปแบบ ABCDEF care bundle ตรวจสอบคุณภาพของเครื่องมือวิจัยโดยผู้ทรงคุณวุฒิ จำนวน 5 ท่าน ได้ค่าดัชนีความตรงตามเนื้อหา เท่ากับ 1 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา Chi-square test, Fisher’s exact test และ Mann-Whitney U test ผลการวิจัย พบว่า วันที่สองของการใช้โปรแกรม กลุ่มทดลองมีค่าคะแนนภาวะสับสนเฉียบพลัน ไม่แตกต่างจากกลุ่มควบคุม แต่วันที่สามกลุ่มทดลองมีค่าคะแนนภาวะสับสนเฉียบพลัน น้อยกว่ากลุ่มควบคุม อย่างมีนัยสำคัญทางสถิติ (U = 637, p < .05)
