ปัจจัยที่มีความสัมพันธ์กับความมั่นใจของสมาชิกครอบครัวผู้ตัดสินใจเลือกการดูแลแบบประคับประคองสำหรับผู้ป่วยที่รับการรักษาในหอผู้ป่วยวิกฤต

บทคัดย่อ

            การวิจัยเชิงพรรณนานี้มีวัตถุประสงค์เพื่อศึกษาความมั่นใจของสมาชิกครอบครัวผู้ตัดสินใจเลือกการดูแลแบบประคับประคองสำหรับผู้ป่วยที่รับการรักษาในหอผู้ป่วยวิกฤตและปัจจัยที่มีความสัมพันธ์ ได้แก่ ความรู้และทัศนคติเกี่ยวกับการดูแลแบบประคับประคอง การได้รับข้อมูล การรับรู้ความต้องการของผู้ป่วย และการสนับสนุนของครอบครัว กลุ่มตัวอย่าง คือ สมาชิกครอบครัวของผู้ป่วยที่มีส่วนร่วมในการตัดสินใจเลือกการรักษาแบบประคับประคอง คัดเลือกตามเกณฑ์ จำนวน 105 คน จากโรงพยาบาลเอกชนเครือโรงพยาบาลพญาไทและเครือโรงพยาบาลเปาโล เก็บข้อมูลด้วยแบบสอบถาม 6 ฉบับ ซึ่งมีค่าดัชนีความตรงตามเนื้อหาทุกฉบับ เท่ากับ 1 ค่าความเที่ยงของแบบสอบถามความรู้เกี่ยวกับการดูแลผู้ป่วยแบบประคับประคอง (KR-20) เท่ากับ 0.64 ค่าสัมประสิทธิ์แอลฟาของครอนบาคของแบบสอบถามการได้รับข้อมูล แบบสอบถามการรับรู้ความต้องการของผู้ป่วย แบบสอบถามการได้รับการสนับสนุนของครอบครัว และแบบประเมินความมั่นใจในการตัดสินใจของครอบครัว เท่ากับ 0.94, 0.98, 0.98 และ 0.91 ตามลำดับ ส่วนแบบสอบถามทัศนคติเกี่ยวกับการดูแลแบบประคับประคอง ด้านอารมณ์ ด้านการรับรู้ และด้านพฤติกรรม มีค่าสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ 0.94, 0.90 และ 0.71 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติสหสัมพันธ์ของเพียร์สัน
            ผลการวิจัย พบว่า สมาชิกครอบครัวของผู้ป่วยมีความมั่นใจในการตัดสินใจเลือกการดูแลแบบประคับประคองอยู่ในระดับปานกลาง (Mean = 10.27, SD = 3.10) โดยพบว่า ความรู้ การได้รับข้อมูล ทัศนคติ และการรับรู้ความต้องการของผู้ป่วยมีความสัมพันธ์ทางบวกกับความมั่นใจในการเลือกการดูแลแบบประคับประคอง อย่างมีนัยสำคัญทางสถิติ (r = .196, r = .225, r = .295, r = .345, p < .05 ตามลำดับ) ขณะที่การสนับสนุนของครอบครัวไม่มีความสัมพันธ์กับความมั่นใจ (r = -.024, p > .05)
            ผลการศึกษาเสนอแนะให้มีการพัฒนาบริการสนับสนุนสมาชิกครอบครัวที่ต้องตัดสินใจเลือกการดูแลแบบประคับประคอง โดยเน้นการเสริมสร้างความรู้และทัศนคติที่ดี การให้ข้อมูลเกี่ยวกับอาการผู้ป่วย รวมถึงมีการวางแผนการดูแลล่วงหน้า เพื่อให้ครอบครัวสามารถตัดสินใจเลือกการดูแลแบบประคับประคองได้อย่างมั่นใจ
