ปัญหาและความต้องการในการพัฒนารูปแบบบริการสุขภาพสำหรับผู้เป็นโรคเบาหวานชนิดที่ 2 ที่ควบคุมระดับน้ำตาลในเลือดไม่ได้ในยุควิถีใหม่

บทคัดย่อ

            การวิจัยเชิงคุณภาพเพื่อศึกษาปัญหาและความต้องการในการพัฒนารูปแบบบริการสุขภาพสำหรับผู้เป็นโรคเบาหวานชนิดที่ 2 ที่ควบคุมระดับน้ำตาลในเลือดไม่ได้ในยุควิถีใหม่ เลือกผู้ให้ข้อมูลแบบเจาะจงจากพยาบาลวิชาชีพที่ปฏิบัติงานคลินิกเบาหวาน 16 ราย และผู้เป็นโรคเบาหวานชนิดที่ 2 ที่ควบคุมระดับน้ำตาลในเลือดไม่ได้ จำนวน 30 ราย จากโรงพยาบาลสังกัดกระทรวงสาธารณสุข 8 แห่ง ในจังหวัดหนึ่งของภาคกลาง ประเทศไทย ใช้การสัมภาษณ์แบบเจาะลึกและวิเคราะห์เนื้อหา
            ผลการวิจัย พบว่า ปัญหาหลักในช่วงการแพร่ระบาดของโรคโควิด 19 ได้แก่ การขาดแคลนอุปกรณ์ บุคลากรไม่เพียงพอทั้งด้านความเชี่ยวชาญและปริมาณ การหยุดให้บริการคลินิก ผู้ป่วยสะท้อนถึงความล่าช้า ความแออัดและการได้รับข้อมูลที่ไม่ตรงกับความต้องการ ส่วนปัจจัยที่ทำให้ไม่สามารถควบคุมระดับน้ำตาลในเลือดได้ ได้แก่ พร่องความรู้เกี่ยวกับการเลือกรับประทานอาหาร รับประทานอาหารที่มีน้ำตาลสูง ขาดความตั้งใจควบคุมอาหาร ขาดการออกกำลังกาย ลักษณะงานเป็นอุปสรรค รับประทานยาไม่ตรงเวลาและไม่ครบ และข้อจำกัดในการจัดหาอาหาร
            ความต้องการในการพัฒนารูปแบบบริการสุขภาพ ได้แก่ การส่งเสริมการจัดการตนเองอย่างต่อเนื่อง ให้บริการที่ลดความเสี่ยงและลดระยะเวลารอคอย เพิ่มบุคลากรที่มีศักยภาพ มีอุปกรณ์ที่เพียงพอ พร้อมใช้ มีสื่อความรู้ที่เข้าใจง่าย ให้บริการด้วยความสุภาพและเป็นมิตร และพัฒนาระบบการติดตามให้คำปรึกษาทางไกล
            ผลการศึกษานี้ช่วยให้เข้าใจบริบท ปัญหาและความต้องการของผู้เป็นโรคเบาหวาน ซึ่งสามารถนำไปใช้พัฒนาระบบบริการสุขภาพที่ตอบสนองยุควิถีใหม่ได้อย่างมีประสิทธิภาพ โดยเน้นการส่งเสริมการจัดการตนเองที่ต่อเนื่อง สะดวก รวดเร็วและสอดคล้องความต้องการของผู้เป็นโรคเบาหวานเพิ่มขึ้น
