บทคัดย่อ
การวิจัยเชิงพรรณนาแบบภาคตัดขวางนี้ มีวัตถุประสงค์เพื่อศึกษาความชุกของการมีฟันถาวรใช้งานได้อย่างน้อย 20 ซี่ และมีคู่สบฟันหลังถาวรอย่างน้อย 4 คู่สบ และศึกษาความสัมพันธ์ระหว่างปัจจัยของโมเดลเชิงนิเวศวิทยา ได้แก่ ปัจจัยระดับบุคคล ระดับระหว่างบุคคล ระดับสถาบัน/องค์กร ระดับชุมชน และระดับนโยบายกับการมีฟันถาวรใช้งานได้อย่างน้อย 20 ซี่ และมีคู่สบฟันหลังถาวรอย่างน้อย 4 คู่สบของผู้สูงอายุในตำบลบางรัก อำเภอเมือง จังหวัดตรัง กลุ่มตัวอย่าง จำนวน 270 คน โดยใช้สูตรคำนวณขนาดตัวอย่างเพื่อการประมาณค่าสัดส่วน กรณีทราบประชากรที่แน่นอน และสุ่มตัวอย่างด้วยวิธีการสุ่มแบบสองขั้นตอน เก็บข้อมูลด้วยแบบตรวจสุขภาพช่องปาก และแบบสัมภาษณ์ ในเดือน มิถุนายน พ.ศ. 2567 วิเคราะห์ข้อมูล โดยใช้สถิติเชิงพรรณนา ได้แก่ จำนวน ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ค่าต่ำสุด และค่าสูงสุด และสถิติเชิงอนุมาน คือ การถดถอยโลจิสติกสำหรับตัวแปรทวิภาค ที่ระดับนัยสำคัญทางสถิติ .05
ผลการศึกษา พบว่า กลุ่มตัวอย่าง ร้อยละ 30.74 ที่มีฟันถาวรใช้งานได้อย่างน้อย 20 ซี่ และมีคู่สบฟันหลังถาวรอย่างน้อย 4 คู่สบ และปัจจัยของโมเดลเชิงนิเวศวิทยาที่มีความสัมพันธ์กับการมีฟันถาวรใช้งานได้อย่างน้อย 20 ซี่ และมีคู่สบฟันหลังถาวรอย่างน้อย 4 คู่สบ (p < .05) คือ ปัจจัยระดับบุคคล ได้แก่ อายุ (p < .001) รายได้ (p = .02) การรับรู้ด้านสุขภาพช่องปากด้านการมีปัญหาในการรับประทานอาหาร หรือบดเคี้ยวอาหาร (p < .001) และการมีปัญหาในการพูด หรือออกเสียง (p < .001) และปัจจัยระดับองค์กร คือ การมีอาสาสมัครสาธารณสุขประจำหมู่บ้าน (อสม.) มาให้คำแนะนำเรื่องการรับประทานอาหาร เพื่อป้องกันฟันผุ (p = .04)
ดังนั้นควรส่งเสริมให้มีการดูแลสุขภาพช่องปากแก่ประชาชนก่อนเข้าสู่ช่วงวัยสูงอายุ เพื่อลดการสูญเสียฟัน และควรได้รับการฟื้นฟูสภาพทางทันตกรรมด้วยการใส่ฟันเทียมทดแทนฟันธรรมชาติที่สูญเสียไป
