ผลของโปรแกรมสัมพันธภาพบำบัดร่วมกับการดูแลต่อเนื่องทางโทรศัพท์ต่อพฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภท

บทคัดย่อ
การวิจัยนี้ใช้แบบแผนการวิจัยกึ่งทดลอง เปรียบเทียบระหว่างสองกลุ่มที่ไม่เท่าเทียมกัน มีวัตถุประสงค์เพื่อเปรียบเทียบ 1) พฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทก่อนและหลังได้รับโปรแกรมสัมพันธภาพบำบัดร่วมกับการดูแลต่อเนื่องทางโทรศัพท์ 2) พฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทระหว่างกลุ่มที่ได้รับโปรแกรมสัมพันธภาพบำบัดร่วมกับการดูแลต่อเนื่องทางโทรศัพท์กับกลุ่มที่ได้รับการพยาบาลตามปกติ กลุ่มตัวอย่างเป็นผู้ป่วยจิตเภทมีคุณสมบัติตามเกณฑ์ที่กำหนดและเข้ารับการรักษาแผนกผู้ป่วยใน ที่สถาบันจิตเวชศาสตร์สมเด็จเจ้าพระยา จำนวน 30 คน ได้รับการจับคู่ให้มีคุณสมบัติใกล้เคียงกันในเรื่อง เพศ และถูกสุ่มเข้ากลุ่ม โดยเป็นกลุ่มทดลอง 15 คน และกลุ่มควบคุม 15 คน เครื่องมือที่ใช้ในการวิจัย ได้แก่ 1) โปรแกรมสัมพันธภาพบำบัดร่วมกับการดูแลต่อเนื่องทางโทรศัพท์ 2) คู่มือการดูแลตนเองหลังจำหน่ายสำหรับผู้ป่วยจิตเภท 3) แบบประเมินสัมพันธภาพระหว่างพยาบาลและผู้ป่วย และ 4) แบบประเมินพฤติกรรมการใช้ยาตามเกณฑ์การรักษา โดยเครื่องมือทุกชุดผ่านการตรวจสอบความตรงของเนื้อหาจากผู้ทรงคุณวุฒิ จำนวน 5 คน เครื่องมือชุดที่ 4 มีค่าความเที่ยงสัมประสิทธิ์แอลฟาของครอนบาค เท่ากับ 0.88 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงบรรยายและการทดสอบ t-test
ผลการวิจัยสรุปได้ ดังนี้
1. ผู้ป่วยจิตเภทในกลุ่มที่ได้รับโปรแกรมสัมพันธภาพบำบัดร่วมกับการดูแลต่อเนื่องทางโทรศัพท์มีพฤติกรรมการใช้ยาตามเกณฑ์การรักษา ในระยะหลังได้รับโปรแกรมสัมพันธภาพบำบัดร่วมกับการดูแลต่อเนื่องทางโทรศัพท์ทันที และหลังได้รับโปรแกรมสัมพันธภาพบำบัดร่วมกับการดูแลต่อเนื่องทางโทรศัพท์ 4 สัปดาห์ ดีกว่าก่อนได้รับโปรแกรมสัมพันธภาพบำบัดร่วมกับการดูแลต่อเนื่องทางโทรศัพท์ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05
2. ผู้ป่วยจิตเภทระหว่างกลุ่มที่ได้รับโปรแกรมสัมพันธภาพบำบัดร่วมกับการดูแลต่อเนื่องทางโทรศัพท์ มีพฤติกรรมการใช้ยาตามเกณฑ์การรักษาดีขึ้นกว่ากลุ่มที่รับการพยาบาลตามปกติ ทั้งในระยะหลังสิ้นสุดโปรแกรมสัมพันธภาพบำบัดร่วมกับการดูแลต่อเนื่องทางโทรศัพท์ทันทีและหลังสิ้นสุดโปรแกรมสัมพันธภาพบำบัดร่วมกับการดูแลต่อเนื่องทางโทรศัพท์ 4 สัปดาห์ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05
