Abstract
Background: Stroke is a major public health problem among older adults, particularly those with hypertensive urgency who are at increased risk of complications and mortality. Effective stroke prevention requires appropriate health behaviors and self-management. This study examined the effects of a motivational enhancement program on stroke prevention behaviors and blood pressure levels among older adults with hypertensive urgency.
Methods: This quasi-experimental study employed a two-group pretest–posttest design. The sample consisted of 78 older adults with hypertensive urgency. Participants were selected using purposive sampling and assigned to an experimental group and a control group, with 39 participants in each group. The research instruments included a stroke prevention behavior questionnaire with a content validity index of 1.00 and a Cronbach’s alpha coefficient of 0.85, a calibrated blood pressure monitor, and a motivation enhancement program based on the concepts of Maddux and Rogers with a content validity index of 0.90. The experimental group received the program for eight weeks, while the control group received usual care. Data were analyzed using descriptive statistics, including frequency, percentage, mean, and standard deviation. The Chi-square test, paired t-tests, and independent t-tests were used for inferential statistical analysis.
Results: After the intervention, the experimental group showed significantly higher stroke prevention behaviors, including diet, physical activity, and medication adherence, than the control group (p<0.001). Both systolic and diastolic blood pressure levels also significantly decreased in the experimental group (p<0.001).
Conclusions: The motivational enhancement program effectively improved stroke prevention behaviors and blood pressure control among older adults with hypertensive urgency. It may be applied as a practical approach to promote health behaviors in older populations.
