การพัฒนาระบบบริการพยาบาลสำหรับผู้ป่วยที่เข้ารับการตรวจวินิจฉัยรักษา ด้วยหัตถการการส่องกล้องโรงพยาบาลน่าน

การวิจัยและพัฒนาครั้งนี้มีวัตถุประสงค์เพื่อพัฒนาและประเมินผลลัพธ์ระบบบริการพยาบาลสำหรับผู้ป่วยที่เข้ารับการตรวจวินิจฉัยรักษาด้วยหัตถการการส่องกล้องโรงพยาบาลน่าน การวิจัยแบ่งออกเป็น 3 ระยะคือ ระยะเตรียมการพัฒนา ระยะดำเนินการพัฒนาระบบ และระยะประเมินผลลัพธ์การพัฒนากลุ่มตัวอย่างประกอบด้วยแพทย์ ทีมสหสาขาวิชาชีพที่เกี่ยวข้องกับการให้บริการตรวจวินิจฉัยรักษาด้วยหัตถการการส่องกล้อง รวมทั้งสิ้น 30 คน ผู้ป่วยที่เข้ารับบริการ ณ หน่วยงานห้องส่องกล้อง 275 คน เครื่องมือประกอบด้วย แบบสำรวจโครงสร้างหน่วยงานห้องส่องกล้อง แบบประเมินคุณภาพการบริการพยาบาลตรวจรักษาพิเศษของกองการพยาบาล แบบสัมภาษณ์แบบมีโครงสร้าง แบบรายงานอุบัติการณ์ แบบสอบถามความพึงพอใจในระบบบริการพยาบาล วิเคราะห์ข้อมูลโดยใช้การวิเคราะห์ประเด็นร่วม การใช้สถิติเชิงพรรณนา และเปรียบเทียบผลลัพธ์การพัฒนาโดยใช้สถิติการเปรียบเทียบ t-test
ผลการศึกษาพบว่า 1.ระยะเตรียมการพัฒนาพบปัญหา 1.1) ด้านโครงสร้างมีปัญหาสถานที่คับแคบพยาบาลวิชาชีพมีอัตรากำลังและศักยภาพไม่เพียงพอ อุปกรณ์เครื่องมือไม่เพียงพอ 1.2) ด้านกระบวนการมีปัญหาแนวทางการส่งต่อไม่ชัดเจนและแนวทางการดูแลผู้ป่วยไม่ครอบคลุม 1.3) ด้านผลลัพธ์มีการให้คำแนะนำขั้นตอนการทำหัตถการเพียงร้อยละ 48.50 อุบัติการณ์การเลื่อนเวลาการทำหัตถการจากการเตรียมผู้ป่วยไม่พร้อม ร้อยละ 14.00 การรับส่งผู้ป่วยผิดพลาดล่าช้า ร้อยละ 8.00 2.ระยะดำเนินการมีการพัฒนาศักยภาพบุคลากร การปรับกระบวนการทำงานเพื่อเพิ่มคุณภาพบริการ การปรับสภาพพื้นที่ในห้องส่องกล้อง และการสนับสนุนทรัพยากรให้เพียงพอ 3. ระยะประเมินผลลัพธ์ พบว่า การเลื่อนเวลาการทำหัตถการจากการเตรียมผู้ป่วยไม่พร้อมและการรับส่งผู้ป่วยผิดพลาดล่าช้ามีจำนวนลดลง ไม่พบอุบัติการณ์ขณะเคลื่อนย้ายผู้ป่วยทั้งผู้ป่วยนอกและผู้ป่วยใน คะแนนความพึงพอใจในบริการพยาบาลห้องส่องกล้องของผู้ป่วยนอกและผู้ดูแลอยู่ในระดับมากถึงมากที่สุด บริการพยาบาลห้องส่องกล้องของพยาบาลวิชาชีพในหอผู้ป่วยและพยาบาลวิชาชีพผู้ประสานงานในโรงพยาบาลชุมชนทุกด้านอยู่ในระดับพึงพอใจมาก และภาพรวมของผู้ป่วยในต่อคุณภาพการบริการหลังพัฒนาสูงกว่าก่อนพัฒนาอย่างมีนัยสำคัญทางสถิติที่ระดับ .01 จึงควรมีการศึกษาสภาพปัญหาการทำงานของระบบบริการพยาบาล และมีการพัฒนาระบบเพื่อแก้ไขปัญหาเพื่อให้เกิดการพัฒนาคุณภาพอย่างต่อเนื่อง
