บทคัดย่อ

โรคธาลัสซีเมียที่มีภาวะเหล็กเกินเป็นโรคเรื้อรังที่ส่งผลกระทบต่อพัฒนาการด้านร่างกาย จิตใจ และสังคมของเด็กในระยะยาว โดยเฉพาะในช่วงวัยเรียนซึ่งเป็นช่วงพัฒนาความรู้สึกมีคุณค่าในตนเอง อย่างไรก็ตาม องค์ความรู้ด้านประสบการณ์ชีวิตเชิงลึกของเด็กกลุ่มนี้ยังมีจำกัด การวิจัยเชิงคุณภาพแนวปรากฏการณ์วิทยานี้มีวัตถุประสงค์เพื่อศึกษาประสบการณ์ชีวิตของผู้ป่วยเด็กโรคธาลัสซีเมียที่มีภาวะเหล็กเกิน ผู้ให้ข้อมูลเป็นเด็กอายุ 10–15 ปี จำนวน 11 ราย คัดเลือกแบบเฉพาะเจาะจงจากคลินิกเด็กโรคธาลัสซีเมียและหอผู้ป่วยเด็ก โรงพยาบาลประจำจังหวัดแห่งหนึ่ง เก็บข้อมูลโดยการสัมภาษณ์เชิงลึก ใช้แนวคำถามแบบกึ่งโครงสร้าง วิเคราะห์ข้อมูลตามแนวทางของ Colaizzi และตรวจสอบความน่าเชื่อถือของข้อมูลด้วยการตรวจสอบแบบสามเส้าและการสะท้อนกลับข้อมูลจนเกิดความอิ่มตัว

ผลการวิจัยพบ 3 ประเด็นหลัก ได้แก่ 1) การตระหนักรู้ต่อโรคและผลกระทบจากภาวะเหล็กเกิน 2) การดำรงชีวิตและการปรับตัวภายใต้ข้อจำกัดของโรคเรื้อรัง และ 3) ความหวัง พลังใจ และแรงสนับสนุนสู่คุณภาพชีวิตที่ดี เด็กมีความเข้าใจโรคตามระดับพัฒนาการ รับรู้ผลกระทบด้านร่างกาย ควบคู่กับความพยายามใช้ชีวิตเช่นเดียวกับเพื่อนวัยเดียวกัน การสนับสนุนจากครอบครัวและทีมสุขภาพมีบทบาทสำคัญต่อการเกิดความหวังและแรงจูงใจในการดูแลตนเอง ผลการศึกษาชี้ให้เห็นว่าพยาบาลควรบูรณาการการดูแลด้านจิตสังคมควบคู่การรักษาทางกาย เพื่อเสริมสร้างความหวัง ความยืดหยุ่นทางจิตใจ และคุณภาพชีวิตของเด็กโรคเรื้อรังอย่างยั่งยืน
