Development of a palliative nursing care system

This research and development design aimed to develop the palliative nursing care system (PNCS) for end-of-life persons and evaluate its effectiveness. Samples were 10 registered nurses (RNs) and 100 patients. Research processes were 1) situation analysis, 2) design and development, 3) implementation, and 4) evaluation. Data collection used a focused group discussion (FDG) and a self-administration questionnaire. Tools were the content record, the FDG guideline, the quality assessment record, the knowledge assessment form, the nursing skill assessment form, the satisfaction questionnaire, and the nursing activity record. The validity showed IOC 0.5-1 and the reliability showed Cronbach’s alpha coefficient 0.76-0.88. Data analysis used content analysis, percent, average, standard deviation, Wilcoxon signed rank test, and Chi square test.
Results showed that a palliative nursing care system (PNCS) is composed of a nursing delivery system as a primary nurse and clinical nursing practice guidelines for palliative care at the end of life. It deployed on the first arrival on admission at the ward until discharged to home. We found that after patients received a PNCS they statistically significant received more valid screening and assessment of their self-care demand, and received more specific planning for palliative care than before at p<.05. RNs had the highest satisfaction on deploying the PNCS. In summary, the PNCS was useful for nurse personnel to provide appropriate care
