The Effects of a Self-Management Program Combined with Social Support from Family on Dyspnea Management Ability and Clinical Outcomes in Readmitted Patients with Chronic Obstructive Pulmonary Disease

Abstract

This quasi-experimental research with a two-group pretest-posttest design aimed to study the effect of a self-management program combined with family social support on dyspnea management ability and clinical outcomes in readmitted chronic obstructive pulmonary disease (COPD) patients. The sample consisted of 60 COPD patients readmitted to the general medicine ward at Mahasarakham Hospital. Subjects were purposively selected according to specified criteria and divided into experimental and control groups of 30 patients each. The experimental tools included a self-management program combined with family social support, a self-management handbook for COPD patients, and a LINE OA application; and the data collection tools were a personal information record form, a clinical outcome record form, a peak expiratory flow rate measurement device, and a self-management behavior questionnaire. Data were analyzed using descriptive statistics, chi-square tests, paired t-tests, independent t-tests, and risk calculations.

The results showed that after program participation, the experimental group had significantly better mean scores for self-management behaviors, dyspnea severity, and peak expiratory flow rate compared to the control group. Additionally, the experimental group reduced their risk of hospital readmission within 28 days by 88%. The findings indicate that the self-management program combined with family social support helps COPD patients improve their self-management behaviors and clinical outcomes, while also reducing the risk of readmission. However, long-term follow-up is recommended.
