The Relationship Between Self-Management Behaviors and Glycosylated Hemoglobin Levels Among Patients with Uncontrolled Diabetes in Urban Communities in the Mueang District of Nakhon Ratchasima Province

Abstract

This descriptive correlational study aimed to investigate the level of self-management behaviors and their relationship with glycated hemoglobin (HbA1c) levels in diabetic patients with uncontrolled blood glucose living in the urban communities of Mueang District, Nakhon Ratchasima Province. A total of 234 participants were selected using convenience sampling. Research instruments included a self-management behavior questionnaire and HbA1c records, with an Index of Item-Objective Congruence (IOC) of 0.91. Data were collected from September to November 2024 and analyzed using frequencies, percentages, means, standard deviations, and Pearson’s correlation coefficients.

Results showed that the overall level of self-management behaviors was low. The highest scoring behavior was complication prevention, followed by medication adherence and medical visits, while dietary management had the lowest score. The five aspects of self-management behaviors, including dietary management, physical activity, medication adherence and medical visits, stress management, and complication prevention, were significantly negatively correlated with HbA1c levels (r = -0.741, -0.530, -0.669, -0.514, and -0.754, respectively; p < .05). These findings suggest that self-management behaviors have a significant impact on blood glucose control in patients with poorly controlled diabetes. Therefore, strategies to enhance patients’ self-management skills should be promoted to improve diabetes management outcomes.
