Development of Nursing Practice Guidelines for Pregnant Women with Preeclampsia with Severe Features and Receiving Magnesium Sulphate.

Abstract

This action research was conducted to develop clinical nursing practice guidelines (CNPG) for pregnant women with preeclampsia with severe features who received magnesium sulphate and to study their effectiveness and outcomes in terms of the service recipients, service providers, and the organization. The research was divided into two phases including 1) the development of CNPG according to the Iowa Model and 2) the implementation of the developed CNPG. The sample consisted of two groups, specifically: 1) thirty pregnant women with preeclampsia with severe features who received magnesium sulphate for treatment and 2) fifteen registered nurses. Assessment of the guidelines by AGREE II standards revealed a guideline quality score of 85.33, and data were collected using a patient knowledge assessment, a nurses' competency assessment, and a satisfaction assessment. The data were analyzed using descriptive statistics and a comparison of the mean knowledge scores of the pregnant women and the mean nursing competency scores of the registered nurses before and after using the CNPG using dependent t-test.

The results were revealed as follows. The CNPG consisted of four phases of nursing practices, i.e., 1) the initial admission phase, 2) the latent phase, 3) the active and labor phase, and 4) the postpartum phase. After CNPG implementation, it was found that 96.70% of the pregnant women had levels of magnesium at the therapeutic level, 100% received symptom distress management, satisfaction with the nursing service was at the highest level, and the mean of the pregnancy knowledge score was significantly higher. In addition, the nurses were satisfied with the CNPG at a high level, the mean score of nursing competency of the registered nurses was significantly higher, and no adverse events among the pregnant women and their fetuses were found (0%).
