Discharge Planning Model for Palliative Care among Patients with End-Stage Head and Neck Cancer: A Systematic Review

Abstract

The quality of the discharge plan for palliative care for head and neck cancer patients can shorten the duration of hospitalization, reduce the cost of care and the burden on the patient’s family, and prevent readmission and promote receiving continuous care. The purpose of this study was to systematically review discharge planning for patients with end-stage head and neck cancer who were receiving palliative care using the framework created by the Joanna Briggs Institute (JBI). The articles reviewed in this study were published in either Thai or English on the Medline, CINAHL, Scopus, ClinicalKey, PsychINFO, ScienceDirect, PubMed, ThaiJO, and ThaiLIS databases between 2011 and 2021. The quality of the studies was assessed using JBI’s Critical Appraisal Tools to facilitate accurate evaluation and data extraction by the research teams. The results found 308 studies which were screened for eligibility and eight studies met the study criteria and were included in the study. The discharge planning model for end-stage head and neck cancer patients who receive palliative care consists of multidisciplinary collaboration, communication between the care team, the patient, and their family using D-METHOD as a teaching tool to prepare patients and caregivers, the demonstration and return-demonstration of required home care skills. Equipment used for teaching the patients and caregivers consisted of medical equipment, manikins, and handbooks on the required home care activities for patients’ and their families. The teaching content included medication information ; wound dressing, preparation for nasogastric tube feeding; observing, detecting, and managing abnormal symptoms and asking for assistance in case of emergency medical situations.
