Development of a clinical nursing practice guideline for neonate receiving therapeutic hypothermia

Abstract

The action research aimed to develop and evaluate clinical nursing practice guidelines (CNPGs) for neonates receiving therapeutic hypothermia in the neonate intensive care unit (NICU). We implemented a three-stage procedure of action research including the first of all, situation analysis to define the clinical problem and intervention, the second, periodically proceeding to generate CNPGs, and the third implementation and continuous improvement. We enroll 15 neonates who received therapeutic hypothermia and 38 nurses from July 2020 - June 2021.

The results were as follows: The CNPGs for neonates receiving therapeutic hypothermia comprised 4 main- guidelines: 1) newborn assessment 2) nursing implementation 3) continuing of care and 4) family information. Nurses were able to comply with CNPGs at 96.18%. Results of CNPGs use showed that the majority of neonates (93.33%) were assessed for eligibility of therapeutic hypothermia intervention within 6 hours after birth. They were assessed and monitored for complications during therapeutic hypothermia. The most complication was the respiratory complication (100%) and the least was the skin injury (40%). The high overall satisfaction rate for CNPGs was high level with a score 3.68. The results suggest that the CNPGs can be used to assess and monitor the complications while neonates receive therapeutic hypothermia.
