Abstract

This action research to development model case management of patient with ST-segment elevation myocardial infarction receiving thrombolytic drugs at Mahasarakham Hospital. The  selected specific patients, including accident and emergency patients. And 34 physician's intensive care units 57 patients with ST-segment elevation myocardial infarction receiving thrombolytic drugs.    By setting additional properties were 2 phases of research in each of 4 phases to Planning, observing, planning and reflecting by the participation of all participants in the research. The assessment for satisfaction qualitative and quantitative data using group discussion. The data analysis quantitative information using quantitative and percentage analysis.Results:  The develop case management model .There are 3 changes 1) Nursing Practical assessment of case manager nurses. The role of nurses in case management 10 cases was correct and complete 92.0%. Evaluation  of  STEMI patient care practice guidelines. To get SK drug of accident and to be honest and complete in all aspects, 86.25 %. 2) Service : Satisfaction of professional nurses of 91.8%.Multidisciplinary satisfaction on case management model 90%.And satisfaction of patients or caregivers. The case management model developed was 92.1 %. Clinical aspects of the quality of care use case management. After used of model case management  for take care of patients patient with ST-segment elevation myocardial infarction and receiving thrombolytic drugs at the Community Hospital were found that this model can have efficiency and can use to be increases the quality for patients care.
