The obesity is one of the major risk factors for chronic disease that requires long-term treatment and high costs. Health promotion is a necessity, which is subject to limited and effective resources. This study of the cost-effectiveness of the health promotion program and the use of self-help groups to prevent obesity was a quasi-experimental research. The sample is village health volunteers with overweight were divided into 2 groups of 50 persons selected by random sampling. The experimental group participated in a health promotion program and the use of self-help groups for 6 months and the control group received regular care. Estimation of cost and body mass index, waist circumference, quality of life before and after research. The cost and effectiveness of the program was calculated by the amount of money that made one sample have a better quality of life. The experimental group had clinical outcome and quality of life different from before intervention and the control group (p < .05). The costs in perspective of provider and 48 service recipients in the experimental group were 31,000 bath and 18,000 baht respectively. The cost of 47 cases in the control group were 3,500 baht and 1,762 baht. 23 persons in experimental group and 3 persons in control group can increase one level quality of life. Cost - effectiveness in experimental group were 1,347.83 baht and 782.61 baht. In the control group were 1,166.67 baht and 587.33 baht respectively. Conclusion: Village health volunteers who participated in the health promotion program and the use of self-help groups to prevent obesity were both cost-effectiveness in the view of providers and service recipients more than non-participants.
