ไขปัญหาและความต้องการในการป้องกันโรคหลอดเลือดสมอง ของผู้มีส่วนได้เสียในการบริการปฐมภูมิ Exploring Problems and Needs of Stroke Care Prevention of Primary Stakeholders

This qualitative research aimed to explore problems and needs of stroke prevention among primary care stakeholders in a primary care unit (PCU) in Mahasarakham province during January to April 2017. Thirty six key informants were selected by using purposive sampling with selection criteria, they were primary care stakeholders from 3 sectors included; 1) people's sector representatives (18), 2) health sector representatives (9) and 3) social sector representatives and other stakeholders (9). Data collecting used multi method such as; in-depth interview, focus group discussion, participatory observation and field recording. Data analyzed by using content analysis.
The research findings include two main themes which were, 1. primary care stakeholders perception in problems
of stroke prevention care services included 3 views from 3 sectors: 1) people sector’s perception in problems of stroke prevention care services included 4 sub-themes there were problems of giving the unclear information of stroke care, proactive services for non-communicable diseases (NCDs) patients in the community not covered, lack of home visits by health providers after discharge and lack of continuity in rehabilitation. 2) social sector’s perception in problems of stroke prevention care services included 2 sub-themes were problems of giving the unclear information of stroke care and work separately 3) health sector’s perception in problems of stroke prevention care services included; clients lack of awareness in primary prevention, the risk groups could not access to service for stroke fast track system and lack of continuity in tertiary prevention. 2. primary care stakeholders’ needs of care services for stroke prevention include 5 sub-themes which were; 1) development of information giving in stroke prevention, 2) improvement of screening and surveillance services for risk groups, 3) promote the community participation in warning signs management and referral system, 4) development of care services of rehabilitation and 5) development of continuing care service. This study guided the PCU to develop care services for stroke prevention within focus on community participation that responds to problems and health care needs under the context of primary care setting.
