บทคัดย่อ

 การวิจัยครั้งนี้เป็นการวิจัยเชิงปฏิบัติการ มีวัตถุประสงค์เพื่อพัฒนาระบบการดูแลผู้ป่วยโรคกล้ามเนื้อหัวใจ ขาดเลือดเฉียบพลันชนิด STEMI โดยใช้รูปแบบการจัดการรายกรณีในโรงพยาบาลขอนแก่น ผู้ร่วมวิจัยประกอบด้วย 2 กลุ่ม คือกลุ่มผู้ร่วมวิจัยหลัก ได้แก่ ทีมพัฒนาระบบการดูแลจำนวน 14 คน และกลุ่มผู้ให้ข้อมูลหลัก ได้แก่ ผู้ป่วยโรค กล้ามเนื้อหัวใจขาดเลือดเฉียบพลันชนิด STEMI จำนวน 15 คน และกลุ่มทีมสหสาขาวิชาชีพที่ดูแลผู้ป่วย จำนวน 50 คน ดำ เนินการวิจั ยเริ่ มตั้ งแต่ เดื อนสิ งหาคม 2551 ถึงกุ มภาพั นธ ์ 2553เก็บรวบรวมข้ อมู ลด้ วยวิ ธี การเชิ งปริ มาณและ เชิงคุณภาพ วิเคราะห์ข้อมูลเชิงคุณภาพ โดยการวิเคราะห์เนื้อหาและสร้างข้อสรุป ข้อมูลเชิงปริมาณใช้สถิติเชิงพรรณนา การแจกแจงความถี่ ค่าเฉลี่ย และร้อยละผลวิจัยการพัฒนาทำให้ได้แนวปฏิบัติของทีมสหสาขาวิชาชีพในการดูแล ผู้ป่วยโรคกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันชนิด STEMI ที่ต้องได้รับยา Streptokinase ความพึงพอใจในการมีส่วนร่วม ต่อการพัฒนารูปแบบการจัดการรายกรณีของทีมสหสาขาวิชาชีพอยู่ในระดับสูง เกิดผลลัพธ์ที่ดีต่อผู้ป่วย ได้แก่ ค่าเฉลี่ย ระยะวันนอนรักษาในโรงพยาบาลลดลงจาก 7.0 วัน เป็น 4.2 วัน ค่าเฉลี่ยระยะเวลานับแต่ผู้ป่วยมาถึงโรงพยาบาลจนได้ รับการรักษาด้วย Streptokinase จาก 126.0 นาที เป็น 29.0 นาที อัตราการกลับมารักษาซํ้าในโรงพยาบาลจากร้อยละ 4.4 เป็น 0 ความพึงพอใจของผู้ใช้บริการ เพิ่มขึ้นจากร้อยละ 77.0 เป็น 95.8 และจากการติดตามเยี่ยมภายหลัง จำหน่ายเมื่อครบ 1 เดือน พบว่า กลุ่มผู้ร่วมวิจัยเลิกสูบบุหรี่ได้ทั้งหมด มีการควบคุมอาหารและการออกกำลังกายดีขึ้น และมีระดับไขมันในเลือดลดลงได้ถึงร้อยละ 80.0 ปัจจัยของความสำเร็จของงานวิจัยครั้งนี้ เนื่องจากรูปแบบการ พยาบาลแบบจัดการรายกรณีทำให้เกิดระบบการดูแลผู้ป่วยอย่างต่อเนื่อง มีการประสานงานความร่วมมือของทีมสห สาขาวิชาชีพที่ดูแลผู้ป่วยร่วมกันและประสานงานการดูแลช่วยเหลือแก้ไขปัญหาเพื่อให้ผู้ป่วยได้รับการดูแลรักษาอย่าง ต่อเนื่องและรวดเร็ว มีการติดต่อสื่อสารอย่างมีประสิทธิภาพ รวมทั้งมีการประเมินและติดตามผลลัพธ์อย่างต่อเนื่อง

This action research was conducted with the aim to develop a case management system for patients with ST-Elevated Myocardial Infarction (STEMI) in Khon Kaen Hospital. Two groups of participants were included in this study. The principal researcher group consisted of 14 health personnel who developed the case management system. The key informant group comprised of 15 patients with STEMI and 50 multidisciplinary health personnel who cared for these patients during hospital admission. The study was carried out during August 2008 to February 2010. Both quantitative and qualitative information were collected during the study period. Qualitative data were examined using content analysis to arrive at the summary. Descriptive statistics were used to analyze quantitative data to obtain frequencies, percentages, and means.

Results indicated positive outcomes associated with the case management system and practice guideline developed to care for patients with STEMI. All health personnel were highly satisfied to participate in this project. In addition, all patients demonstrated desirable outcomes after implementing the case management system. Compared to the time before the project implementation, the average time of hospital stay decreased from 7.0 to 4.2 days. All patients received treatments with streptokinase within the recommended period with the average door-to-needle time decreased from 126.0 minutes to 29.0 minutes. The rate of re-admission with the same condition (within 28 days) also dropped from 4.4 percent to none. In addition, the average patient satisfaction with health services increased from 77.0 to 95.8 percents. In addition, home visits one month after discharge revealed that all patients had stopped smoking, increased regular exercise, and adopted better dietary control. Most patients (80%) had decreased levels of blood cholesterol. The success observed in this study in managing the care for patients with STEMI was attributable to the case management system that enabled continuing care, coordinative efforts among the interdisciplinary team members, effective communication and ongoing assessment and outcome evaluation. These in turn led to prompt and efficient management of the patients’ problems and healthcare needs.
