ผลของรูปแบบการพยาบาลเพื่อเตรียมผู้ป่วยผ่าตัดต้อกระจก ต่อความวิตกกังวล ความร่วมมือในการผ่าตัด และอัตราการงดหรือเลื่อนผ่าตัด

ความวิตกกังวลก่อนการผ่าตัดต้อกระจกเป็นภาวะคุกคามที่ส่งผลให้ผู้ป่วยขาดความร่วมมือและนำไปสู่ปัญหาการงดหรือเลื่อนผ่าตัด การวิจัยกึ่งทดลองนี้จึงมีวัตถุประสงค์เพื่อศึกษาผลของรูปแบบการพยาบาลเพื่อเตรียมผู้ป่วยผ่าตัดต้อกระจก (Information-Preparation-Decrease Anxiety and Complication: IPD model) ต่อ 1)ระดับความวิตกกังวล 2) ความร่วมมือในการผ่าตัด และ 3) อัตราการงดหรือเลื่อนผ่าตัด กลุ่มตัวอย่างคือผู้ป่วยต้อกระจกที่เข้ารับการผ่าตัดในโรงพยาบาลมุกดาหาร จำนวน 60 คน เลือกแบบเจาะจง แบ่งเป็นกลุ่มทดลอง 30 คนที่ได้รับรูปแบบการพยาบาล IPD model และ กลุ่มควบคุม 30 คน ที่ได้รับการพยาบาลตามมาตรฐานปกติ เครื่องมือที่ใช้ ได้แก่ รูปแบบการพยาบาล IPD model และแบบประเมินความร่วมมือในการผ่าตัดผ่านการตรวจสอบความตรงเชิงเนื้อหา เท่ากับ 0.92 และ 0.90 ตามลำดับ ส่วนแบบประเมินความวิตกกังวล (The Amsterdam preoperative anxiety and information scale: APAIS) และแบบประเมินความร่วมมือฯ มีค่าความเชื่อมั่นเท่ากับ 0.90 และ 0.89 ตามลำดับ และ แบบบันทึกการงดหรือเลื่อนผ่าตัดมาตรฐานของโรงพยาบาล วิเคราะห์ข้อมูลด้วยสถิติ Chi-square test, Independent t-test, Paired t-test และ Fisher’s Exact Test
การวิจัยพบว่า ภายหลังการทดลองกลุ่มทดลองมีคะแนนเฉลี่ยความวิตกกังวลลดลงอย่างมีนัยสำคัญทางสถิติขณะที่กลุ่มควบคุมไม่มีการเปลี่ยนแปลง เมื่อเปรียบเทียบระหว่างกลุ่มพบว่า กลุ่มทดลองมีคะแนนเฉลี่ยความวิตกกังวลต่ำกว่าและมีพฤติกรรมความร่วมมือขณะผ่าตัดสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ สำหรับอัตราการงดหรือเลื่อนผ่าตัด แม้ไม่พบความแตกต่างทางสถิติแต่มีนัยสำคัญเชิงคลินิก โดยกลุ่มทดลองไม่พบอุบัติการณ์ ในขณะที่กลุ่มควบคุมพบร้อยละ 13.33 สรุปได้ว่ารูปแบบ IPD Model มีประสิทธิผลในการลดความวิตกกังวลและส่งเสริมความร่วมมือของผู้ป่วย ข้อเสนอแนะควรพัฒนาแนวทาง การพยาบาลที่เน้นการสื่อสารด้วยภาษาถิ่น การฝึกทักษะในสถานการณ์จำลอง และการคัดกรองความเสี่ยงเชิงรุกผ่านระบบติดตามทางไกล เพื่อยกระดับประสิทธิภาพการบริหารจัดการห้องผ่าตัดและผลลัพธ์การรักษาที่ดี
