(บทความวิจัย) การกำหนดช่วงอ้างอิงของการทดสอบ Dihydrorhodamine 123 โดยใช้เครื่อง BD FACSLyric ในประชากรไทย สำหรับการวินิจฉัยโรค Chronic Granulomatous Disease

โรค chronic granulomatous disease (CGD) เป็นความบกพร่องทางภูมิคุ้มกันแต่กำเนิดที่เกิดจากความผิดปกติของเอนไซม์ NADPH oxidase ส่งผลให้ร่างกายไม่สามารถสร้างสารอนุมูลอิสระในช่วง oxidative burst ซึ่งกลไกนี้มีความจำเป็นอย่างยิ่งในการกำจัดเชื้อแบคทีเรียและเชื้อรา ทำให้มีการติดเชื้อซ้ำและเกิดแกรนูโลมา (granuloma) ในหลายอวัยวะของร่างกาย การทดสอบมาตรฐานที่ช่วยในการวินิจฉัยโรค CGD คือ ไดไฮโดรโรดามีน 123 (dihydrorhodamine 123; DHR assay) ด้วยเทคนิคโฟลไซโตเมทรีซึ่งเป็นวิธีที่รวดเร็ว ง่ายและมีความไวสูง การทดสอบนี้ให้บริการโดยห้องปฏิบัติการทางด้านเซลล์และอณูชีววิทยา ภาควิชาวิทยาภูมิคุ้มกัน คณะแพทยศาสตร์ศิริราชพยาบาลมานานกว่า 10 ปี แต่ไม่เคยมีรายงานถึงค่าอ้างอิงในกลุ่มประชากรไทย การศึกษานี้มีวัตถุประสงค์เพื่อกำหนดช่วงอ้างอิงของการทดสอบ DHR assay ด้วยเครื่อง BD FACSLyric สำหรับใช้เป็นแนวทางในการแปลผลทางห้องปฏิบัติการ โดยรวบรวมผลการตรวจวิเคราะห์จากตัวอย่างคนปกติ จำนวน 285 ราย ระหว่างปี พ.ศ. 2562-2566 ผลการศึกษาพบว่า ช่วงอ้างอิง (2.5th-97.5th percentiles) ของค่า stimulation index (SI) อยู่ในช่วง 105-304 ค่ามัธยฐานของความเข้มการเรืองแสง (median fluorescence intensity; MFI) ของตัวอย่าง unstimulated อยู่ในช่วง 12-39 และของตัวอย่าง PMA-stimulated อยู่ในช่วง 2,631-6,664 โดยกำหนดค่า SI เท่ากับ 105 ซึ่งเป็นค่าที่ตำแหน่ง 2.5th percentile เป็นค่า cut-off สำหรับการแปลผล เมื่อเปรียบเทียบกับกลุ่มผู้ป่วย พบว่าการทดสอบ DHR assay มีความไว 100% และความจำเพาะ 97.5% นอกจากนี้ยังพบรูปแบบความผิดปกติของผลการทดสอบในผู้ป่วยซึ่งช่วยสนับสนุนการวินิจฉัยโรค CGD ได้อย่างมีประสิทธิภาพ
