บทคัดย่อ

แอนติบอดีต่อแอนติเจนบนผิวเม็ดเลือดแดงต่างหมู่ (alloimmunization) นอกระบบ ABO โดยเฉพาะแอนติเจนในระบบ Rh (E, c) และระบบ MNS (Mia) เป็นภาวะแทรกซ้อนสำคัญในผู้ป่วยที่รับโลหิตเป็นประจำ อาจนำไปสู่ปฏิกิริยาแตกทำลายของเม็ดเลือดแดงทั้งแบบฉับพลันและแบบล่าช้า จากการสำรวจข้อมูลย้อนหลังของโรงพยาบาลพระอาจารย์มั่น ภูริทัตโต พบอัตราการเกิดแอนติบอดีต่อแอนติเจนบนผิวเม็ดเลือดแดงต่างหมู่ ร้อยละ 2.38 จึงมีความจำเป็นในการจัดทำฐานข้อมูลความชุกของแอนติเจนและประเมินประสิทธิภาพของการให้โลหิตแบบจับคู่แอนติเจน เป็นการศึกษาข้อมูลย้อนหลังจากผู้ป่วยที่รับโลหิตประจำ 180 ราย ณ โรงพยาบาลพระอาจารย์มั่น ภูริทัตโต ระหว่างเดือนตุลาคม พ.ศ. 2567 ถึงเดือนมีนาคม พ.ศ. 2568 แบ่งเป็นผู้ป่วยธาลัสซีเมีย 28 ราย โรคไตเรื้อรัง 12 ราย โรคไตวายระยะสุดท้าย 5 ราย และโลหิตจางเรื้อรังจากโรคอื่น 135 ราย ตรวจหาแอนติเจน E, c และ Mia ด้วยวิธีหลอดทดลองมาตรฐาน และติดตามการสร้างแอนติบอดีด้วยการตรวจคัดกรองแอนติบอดี ที่ 1, 3 และ 6 เดือน หลังได้รับโลหิตแบบจับคู่แอนติเจน E, c และ Mia ผลการศึกษา พบให้โลหิตแบบจับคู่แอนติเจนรวม 175 ครั้ง (204 ยูนิต) ตลอดระยะเวลาติดตาม 6 เดือน ไม่พบการเกิดแอนติบอดีต่อแอนติเจนบนผิวเม็ดเลือดแดงต่างหมู่รายใหม่ (0/180 ราย) ความชุกของแอนติเจน E, c และ Mia ในผู้ป่วยทั้งหมดเท่ากับร้อยละ 33.3 (60 ราย) ร้อยละ 25.6 (46 ราย) และร้อยละ 21.1 (38 ราย) ตามลำดับ ผู้ป่วยธาลัสซีเมียมีความชุกของแอนติเจน c และ Mia ต่ำกว่ากลุ่มอื่น คือร้อยละ 14.3 (4/28) และร้อยละ 3.6 (1/28) ตามลำดับ ผลการศึกษาสนับสนุนการให้โลหิตแบบจับคู่แอนติเจน E, c และ Mia มาใช้เป็นมาตรฐานในการดูแลผู้ป่วยที่รับโลหิตเป็นประจำ เพื่อลดความเสี่ยงต่อการเกิดแอนติบอดีต่อแอนติเจนบนผิวเม็ดเลือดแดงต่างหมู่
