บทคัดย่อ

การติดเชื้อร่วมระหว่างวัณโรคและเอชไอวี (TB-HIV co-infection) ถือเป็นภาระทางสาธารณสุขระดับโลกที่สำคัญ โดยอุบัติการณ์การเสียชีวิตที่เกี่ยวข้องกับวัณโรคในผู้ติดเชื้อเอชไอวีสูงถึงประมาณ 187,000 รายต่อปี อย่างไรก็ตาม ตัวบ่งชี้ทางชีวภาพ (biomarkers) ในปัจจุบันยังคงมีข้อจำกัดด้านความไวและความจำเพาะในการตรวจวินิจฉัยวัณโรคระยะแสดงอาการ (active tuberculosis) ในกลุ่มผู้ติดเชื้อเอชไอวี การศึกษานี้จึงได้ดำเนินการวิเคราะห์แบบบูรณาการของข้อมูลทรานสคริปโตมิกส์ (transcriptomic data) จากชุดข้อมูลอิสระ 4 ชุด รวมกลุ่มตัวอย่างทั้งหมด 670 ตัวอย่าง ประกอบด้วย ผู้ป่วยติดเชื้อร่วม TB-HIV 185 ราย ผู้ติดเชื้อเอชไอวีเพียงอย่างเดียว 162 ราย ผู้ป่วยวัณโรคเพียงอย่างเดียว 179 ราย และกลุ่มควบคุมที่มีสุขภาพดี 144 ราย จากประเทศแอฟริกาใต้ ยูกันดา สหราชอาณาจักร และอินเดีย จากการควบคุมคุณภาพอย่างเข้มงวดและการปรับแก้ผลกระทบจากชุดข้อมูล (batch effect correction) ด้วยอัลกอริทึม ComBat (sva package v3.46.0, R v4.3.0) การศึกษาได้ทำการวิเคราะห์การแสดงออกของยีนที่แตกต่างกัน (differential expression analysis) ด้วยวิธี limma และการเพิ่มพูนกลุ่มยีน (Gene Set Enrichment Analysis: GSEA) ตามเกณฑ์มาตรฐานของสถาบัน Broad Institute ผลการวิเคราะห์พบตัวบ่งชี้ทางชีวภาพที่มีความแม่นยำสูงจำนวน 34 ชนิด ซึ่งให้ค่าพื้นที่ใต้กราฟ ROC (AUC) สูงถึง 0.947 (95% CI: 0.923-0.971) โดยมีความไวร้อยละ 88.9 และความจำเพาะร้อยละ 93.6 นอกจากนี้ การวิเคราะห์ GSEA ยังเผยให้เห็นเส้นทางชีวภาพ (biological pathways) ที่มีการเพิ่มพูนอย่างมีนัยสำคัญจำนวน 47 เส้นทาง (FDR < 0.001) โดยเฉพาะการตอบสนองต่ออินเตอร์เฟียรอนแกมมา (interferon gamma response) ที่มีค่า NES เท่ากับ 2.89 และกระบวนการสลายทริปโตเฟน (tryptophan catabolism) ที่มีค่า NES เท่ากับ 2.67 โดยพบยีน IDO1 เป็นจุดตรวจสอบทางเมตาบอลิซึม (metabolic checkpoint) ที่สำคัญ การวิเคราะห์โครงสร้างเครือข่ายความสัมพันธ์ (network topology analysis) ระบุเป้าหมายการรักษาที่มีความเชื่อมั่นสูง 12 ตำแหน่ง รวมถึง CXCL10 และ IDO1 ทั้งนี้ ผลการตรวจสอบความถูกต้องข้ามชุดข้อมูลอิสระ (cross validation) ยืนยันถึงความแม่นยำในการจำแนกกลุ่มตัวอย่างได้สูงถึงร้อยละ 91.2
