บทคัดย่อ

การให้บริการจุดเจาะเลือดในโรงพยาบาลจำเป็นต้องมีความรวดเร็วและแม่นยำเพื่อสนับสนุนกระบวนการวินิจฉัยและการรักษาผู้ป่วยอย่างมีประสิทธิภาพ อย่างไรก็ตาม ระบบเดิมยังคงประสบปัญหา เช่น ความล่าช้า ความผิดพลาดในการเตรียมหลอดเก็บเลือด และการรอคอยในหลายจุดบริการ งานวิจัยนี้มีวัตถุประสงค์เพื่อพัฒนาระบบจุดเจาะเลือดแบบ One Stop Service โดยใช้ระเบียบวิธีวิจัยและพัฒนา (Research and Development) ร่วมกับแนวคิด Lean และการวิเคราะห์กระบวนการด้วย Value Stream Mapping (VSM) เพื่อลดความซ้ำซ้อน เพิ่มคุณค่าในกระบวนการ และสร้างความพึงพอใจให้กับผู้รับบริการ กลุ่มตัวอย่างคือผู้รับบริการเจาะเลือดผู้ป่วยนอกก่อนพบแพทย์ เวลา 07.30-09.00 น. ในวันและเวลาราชการ ณ โรงพยาบาลลำพูน จังหวัดลำพูน โดยแบ่งช่วงเวลาเก็บข้อมูลเป็นก่อนการปรับปรุง (ตุลาคม 2566 - พฤศจิกายน 2567) และหลังการปรับปรุง (ธันวาคม 2567 - กุมภาพันธ์ 2568) ผลการศึกษาเปรียบเทียบก่อนและหลังการปรับปรุงพบว่า จำนวนขั้นตอนลดลงจาก 7 เหลือ 5 ขั้นตอน เวลาที่ใช้ในกระบวนการลดลงจาก 143 นาที เหลือ 97 นาที ระยะเวลารอคอยลดลงจาก 55 นาที เหลือ 13 นาที ค่า Total First Time Quality (FTQ) เพิ่มจากร้อยละ 94 เป็นร้อยละ 99 และ Value Added เพิ่มจากร้อยละ 47 เป็นร้อยละ 84 นอกจากนี้ อัตราความปลอดภัยในกระบวนการเจาะเลือดเพิ่มขึ้นจากร้อยละ 99.6 เป็นร้อยละ 99.9 และความพึงพอใจของผู้ป่วยเพิ่มขึ้นจากร้อยละ 87 เป็นร้อยละ 93 ผลการศึกษาชี้ให้เห็นว่าการพัฒนาระบบจุดเจาะเลือดแบบ One Stop Service สามารถลดความสูญเปล่า เพิ่มประสิทธิภาพการให้บริการ และสามารถใช้เป็นต้นแบบในการปรับปรุงระบบในโรงพยาบาลอื่นในอนาคตได้อย่างมีประสิทธิผล
