บทคัดย่อ

วัตถุประสงค์ : เพื่อพัฒนาและประเมินผลรูปแบบการดูแลผู้ป่วยจิตเวชกลุ่มเสี่ยงสูงต่อการก่อความรุนแรง (serious mental illness with high risk to violence: SMI-V) ในชุมชน

วิธีการ : การวิจัยเชิงปฏิบัติการแบบผสานวิธี ในอำเภอแกดำ จังหวัดมหาสารคาม ในกลุ่มผู้ให้บริการ 20 คน ภาคีเครือข่ายในชุมชน 82 คน ผู้ป่วย SMI-V 8 คน และญาติผู้ดูแล 8 คน แบ่งเป็น 3 ระยะ ระยะที่ 1 การศึกษาสถานการณ์และวิเคราะห์ปัญหา ระยะที่ 2 การพัฒนารูปแบบการดูแลตามวงจร PAOR และระยะที่ 3 การประเมินผลและสะท้อนผลการดำเนินงาน ได้แก่ ความรู้และการมีส่วนร่วมของภาคีเครือข่าย ภาระของผู้ดูแล และอาการทางจิตเวช

ผล : ในพื้นที่พบแนวโน้มผู้ป่วย SMI-V เพิ่มขึ้น ปัญหาความต่อเนื่องของระบบดูแลติดตามและส่งต่อข้อมูล ทัศนคติและการตีตราทางสังคม และการขาดความรู้และสมรรถนะของบุคลากรและญาติผู้ดูแล ซึ่งส่งผลต่อการกำเริบซ้ำและความรุนแรงในชุมชน จึงพัฒนาเป็น KAEDAM Model ซึ่งมีองค์ประกอบ 6 ด้าน ได้แก่ การพัฒนาศักยภาพเครือข่าย การเพิ่มการเข้าถึงบริการ การเสริมพลังผู้ป่วยและผู้ดูแล การป้องกันภาวะซึมเศร้า การฟื้นฟูสมรรถภาพในชุมชน และการติดตามประเมินผลเชิงนโยบาย ภายหลังดำเนินการ กลุ่มภาคีเครือข่ายในชุมชนมีคะแนนความรู้และการมีส่วนร่วมเพิ่มขึ้น ญาติผู้ดูแลมีภาระของผู้ดูแล ภาวะซึมเศร้า และความเครียดลดลง และกลุ่มผู้ป่วยมีอาการทางจิต ภาวะซึมเศร้า ความเครียด และความเสี่ยงฆ่าตัวตายลดลงอย่างมีนัยสำคัญทางสถิติ โดยทุกคนมีความเสี่ยงโดยรวมลดลงเป็นระดับต่ำ (สีเขียว) ภายในระยะเวลาติดตาม 1 ปี

สรุป : KAEDAM Model มีแนวโน้มเพิ่มความรู้และการมีส่วนร่วมของภาคีเครือข่ายในชุมชน และลดภาระของผู้ดูแลและอาการทางจิตเวชของผู้ป่วย ทั้งนี้ ควรมีการศึกษาเพิ่มเติมโดยมีกลุ่มเปรียบเทียบและติดตามผลระยะยาวเพื่อยืนยันถึงประสิทธิผลและความยั่งยืนของรูปแบบ
