ปัจจัยที่เกี่ยวข้องกับภาวะซึมเศร้าในผู้ดูแลผู้ป่วยจิตเวชที่แผนกผู้ป่วยนอก โรงพยาบาลปัตตานี

วัตถุประสงค์ : เพื่อหาความชุกและปัจจัยที่เกี่ยวข้องกับภาวะซึมเศร้าในผู้ดูแลผู้ป่วยจิตเวช
วิธีการ : การศึกษาเชิงวิเคราะห์แบบตัดขวางในผู้ดูแลผู้ป่วยจิตเวชที่คลินิกจิตเวช โรงพยาบาลปัตตานี ตั้งแต่ 1 ธันวาคม พ.ศ. 2566 ถึง 31 มกราคม พ.ศ. 2567 เครื่องมือที่ใช้ ได้แก่ แบบเก็บข้อมูลทั่วไปและแบบประเมินภาวะซึมเศร้า (patient health questionire: PHQ-9) ฉบับภาษาไทย วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติเชิงอนุมาน (chi-square หรือ Fisher’s exact test) และการถดถอยโลจิสติก
ผล : กลุ่มตัวอย่าง 400 คน พบความชุกภาวะซึมเศร้า (PHQ-9 ≥ 9 คะแนน) ร้อยละ 16.5 ปัจจัยที่มีความสัมพันธ์กับภาวะซึมเศร้าอย่างมีนัยสำคัญทางสถิติ ได้แก่ อายุ 21 - 30 ปี (AOR = 0.16, 95% CI = 0.03 - 0.81) อายุ 41 - 50 ปี (AOR = 0.11, 95% CI = 0.02 - 0.66) และอายุ 51 - 60 ปี (AOR = 0.08, 95% CI = 0.01 - 0.55) เมื่อเทียบกับอายุ 18 - 20 ปี สถานภาพหย่าร้าง/แยกกันอยู่ (AOR = 4.36, 95% CI = 1.65 - 11.47) เมื่อเทียบกับสถานภาพสมรส การศึกษาระดับประถมศึกษา (AOR = 4.84, 95% CI = 1.54 - 15.22) และมัธยมศึกษา (AOR = 3.38, 95% CI = 1.31 - 8.70) เมื่อเทียบกับระดับปริญญาตรี อาชีพค้าขาย/ธุรกิจส่วนตัว (AOR = 3.14, 95% CI = 1.31 - 7.49) และรับราชการ (AOR = 7.18, 95% CI = 1.92 - 26.82) เมื่อเทียบกับอาชีพรับจ้าง และรายได้ครอบครัวไม่เพียงพอกับรายจ่าย (AOR = 2.19, 95% CI = 1.18 - 4.06) เมื่อเทียบกับรายได้เพียงพอ/เหลือเก็บ
สรุป : ควรมีการคัดกรองและดูแลรักษาสุขภาพจิตของผู้ดูแลผู้ป่วยจิตเวช โดยเฉพาะผู้ดูแลมีภาระหน้าที่อื่นร่วมด้วยและมีข้อจำกัดทางเศรษฐกิจและสังคม
