ความสัมพันธ์ระหว่างผลกระทบจากเหตุการณ์รุนแรงในวัยเด็กของมารดาและวิธีการในการเลี้ยงดูลูก

บทคัดย่อ

วัตถุประสงค์ : เพื่อศึกษาความสัมพันธ์ระหว่างเหตุการณ์รุนแรงในวัยเด็ก (adverse childhood experiences; ACEs) ของมารดากับทักษะในการเลี้ยงลูก รวมถึงปัจจัยป้องกันและปัจจัยเสี่ยงของการเลี้ยงดูลูกด้วยความรุนแรงในมารดาที่มี ACEs

วิธีการ : เป็นการศึกษาแบบ cross sectional study ในมารดาที่มีอายุ 18 ปีขึ้นไป ตอบแบบสอบถามด้วยตนเองจำนวน 4 ฉบับ คือ ข้อมูลพื้นฐาน เหตุการณ์รุนแรงในวัยเด็ก การจัดการพฤติกรรมเด็ก และการเลี้ยงดูเด็กเชิงบวก

ผล : จากมารดาจำนวน 519 คนที่ตอบแบบสอบถาม มี 150 คนมีประวัติ ACEs มารดาที่มีประวัติ ACEs มีปัญหาชีวิตสมรสมากกว่าและความเพียงพอของรายได้ต่ำกว่าอย่างมีนัยสำคัญ (p = .001) หลังจากควบคุมปัจจัยการศึกษา รายได้ และความเพียงพอของรายได้ พบว่า มารดาที่มี ACEs มีสัมพันธภาพที่ดีกับสามีต่ำกว่า (AOR = 0.4) มีปากเสียงกับสามี (AOR = 2.04) ลงไม้ลงมือกับสามี (AOR = 2.57) สามีดื่มสุรา (AOR = 1.47) และใช้สารเสพติด (AOR = 18.8) มากกว่า มีทักษะการเลี้ยงลูกเชิงบวกต่ำกว่า (AOR = 0.55) และจัดการพฤติกรรมเด็กด้วยความรุนแรงมากกว่ากลุ่มที่ไม่มี ACEs  (AOR = 3.74) ปัจจัยที่ลดการจัดการปัญหาพฤติกรรมเด็กด้วยความรุนแรงในกลุ่มมารดาที่มี ACEs คือ การอยู่กับสามีที่จดทะเบียน และอยู่ในครอบครัวที่สามารถปรึกษาหารือกัน หรือพึ่งพากันได้

สรุป : เหตุการณ์รุนแรงในวัยเด็กของมารดามีความสัมพันธ์กับชีวิตสมรสและทักษะการดูแลบุตร การอยู่กับสามีที่จดทะเบียน สามารถปรึกษาหารือกัน และพึ่งพากันได้ ลดโอกาสการใช้รุนแรงในการจัดการพฤติกรรมเด็กในมารดากลุ่มนี้
