This study was to examine the association between the risk assessment by psychiatrists, SAFE precaution (Suicide, Accident, Fighting and Escape), Brief Psychiatric Rating scale( BPRS) and the actual violence during first two weeks admission. The medical records of 421 in- patients admitted to Srithaya Hospital during December 2004 were analysed. There were 60.6 % male, 63.5% diagnosed of schizophrenia. The actual violence within first 2 weeks of hospitalization was 20.9 %. Patients , who had been positive assessed by psychiatrist and more than 13 score of BPRS on hostile, uncooperativeness , suspiciousness item , developed violence 10 times (OR = 10.5 , 95 % CI = 3.8-29.0 ) and 1.7 times(OR = 1.7 , 95 % CI = 1.0 – 3.0) higher than those without risks. In conclusion, risk assessment for violence in acute psychiatric patients predicted by psychiatrist was more reliable than bedside psychometric tests.
