Adverse drug reaction surveil lance: Srithanya’s experience.

บทคัดย่อ

Objective: To develop and evaluation adverse drug reaction surveillance.

Materials and methods :   This research was developed from routine work by Adhoc team. The  activities  included  group  discussion, HOIR  and  documents  analysis  to  develop  the ADR surveillance system. A pilot project, data collecting and synthesis for continuous quality improvement had conducted from January 2004  to June 2005.

Results: The surveillance system development was divided  into 5 stages by which  the first two focused on system design and a record form version1. The third phase adjusted the recording system for physician and pharmacists and modified the record form to  the version2 consisting of each symptom of ADR. The 4th phase applied the record form to the version 3 classified common and
severity of ADR on the back page of a record form and modified a table format for nurse. Drug interaction  list was changed.  The 5th phase  improved the  intervention for nurse and physician to ADR according  to the severity. In addition, drug  lists, recording system for  in-patient and out-patient were adjusted all phases.

Conclusions: Surveillance ADR system of Srithanya Hospital was established. It has been a learning process needed collaborations from multidisciplinary teams and  HOIR analysis to continue a quality  improvement  regarding system design, drugs  list  and multidisciplinary  role  and proper intervention.
