The effect of clozapine and factors related periodontal status in patients with schizophrenia

Objective  To compare periodontal status between patients with schizophrenia who taking clozapine and typical antipsychotics. And to study the factors that effect the periodontal status on them.

Materials and methods The cross-sectional study was performed among inpatient cases with schizophrenia who were received antipsychotics longer than 12 months admitted to Srithunya Hospital from 1 June till 15 July 2013. The use of antipsychotics of participants were recorded, and the BMI classification was done in accordance with Asian standard scales. Interview administered questionairs related to risk factors of metabolic syndrome and periodontitis. Oral examinations were measured the following indices: plaque index(PI); bleeding on probing (BOP); clinical attachment level (CAL); probing depth (PD) and decayed, missing and filled teeth (DMFT). The data were compared by one-way ANOVA, t-test, correlation and multiple linear regression.

Results  A total 105 schizophrenic inpatients participated in this study. There were 3

groups of antipsychotics i.e. group-1: 31 participants was received clozapine monotherapy; group-2: 25 participants was received clozapine polypharmacy and group-3: 49 participants was received typical antipsychotics. Among 3 groups: PI, BOP, CAL, PD, Missing teeth, BMI and waist circumference, were not significant. The risk factors that effect the periodontal status were overweight, male gender, aging and duration of illness. The protective factors were candy eating and long stay in the hospital.

Conclusion Periodontal disease was severe and frequently occurred in schizophrenic patients. The use of clozapine was not significantly associated with oral hygiene, periodontal status and tooth loss.
