Signal Detection of Adverse Events Associated with Herbal and Traditional Medicines in Thailand: a Retrospective Analysis of Thai Vigibase between 1998 and 2020

Abstract
Background: Thailand has prioritized the development of herbal and traditional medicine
under the national vision: Thai Herbs for Health Security and Sustainable Economic
Development. This policy has contributed to a substantial increase in the use of herbal
and traditional medicine services nationwide. Accordingly, the importance of safety
surveillance in the use of these products has grown. Signal detection, as the initial phase
of signal management, is essential for generating preliminary safety data that may guide
the development of pharmacovigilance strategies, risk communication, and appropriate
risk management interventions.
Objectives: To identify potential safety signals of adverse events (AEs) associated with
herbal and traditional medicines.
Methods: An analytical study was conducted using reports from January 1998 to August
2020. A total of 1,688 reports involving 2,822 drug-AE pairs were analyzed. Potential
signals were identified based on four criteria: 1) at least three reports of the same drug-AE
pair; 2) reporting odds ratio (ROR) and the lower bound of its 95% confidence interval (CI)
greater than 1; 3) the AE classified by WHO as serious or fatal; and 4) a positive percentage
change in ROR compared to the previous 6 months.
Results: Adverse events associated with the following herbs were observed: Andrographis
paniculata (i.e., anaphylactic shock), Curcuma longa (i.e., eructation), Derris scandens
(i.e., polyuria), Ya Sahatsara (i.e., urinary retention), and Cissus quadrangularis (i.e., abnormal
feces). Two herb-AE pairs met all four criteria, indicating potential safety signals: Andrographis
paniculata–anaphylactic shock (ROR=2.32, 95%CI: 1.03–5.21) and Derris scandens–polyuria
(ROR=5.12, 95%CI: 1.73–15.14). No potential signals were found for Curcuma longa,
Ya Sahatsara, or Cissus quadrangularis.
Conclusions: This study identified potential safety signals for certain herbal and traditional
medicines. These findings can inform ongoing pharmacovigilance and risk management
strategies in Thailand.
