The Effects of Self-management Program on Fluid Overload in End-stage Renal Disease Patients Receiving Hemodialysis at Chiangrai Prachanukroh Hospital

This quasi-experimental research aims to study the effects of a self-management program on fluid overload in end-stage chronic kidney disease patients undergoing dialysis. It compares self-management behaviors related to fluid overload before and after receiving the self-management program in the experimental group, self-management behavior scores between the experimental and control groups, and interdialytic weight gain (IDWG) between the experimental and control groups. The sample consisted of 70 patients aged 18 years and older undergoing hemodialysis, selected through purposive sampling, matched by basic characteristics, and randomly assigned to either the experimental group or the control group, with 35 participants in each group. The experimental group received the self-management program, while the control group received standard nursing care for 4 weeks. Research instruments included the self-management program, general information recording form, fluid overload recording form, and self-management behavior questionnaire. Content validity was confirmed with CVI values of 0.9 and 0.89, and the questionnaire's reliability was 0.90. Data were analyzed using descriptive statistics, paired t-tests, independent t-tests.
The study results showed that after receiving the program, the experimental group had a significantly higher self-management score for fluid overload compared to the control group (t = 22.84, p< .001) and a significantly lower IDWG than the control group (t = 3.19, p=.002, 95% CI =-0.63, -0.15). Self-management behaviors in specific areas revealed that the experimental group had better medication adherence, fluid restriction, and dietary consumption than the control group. In conclusion, the self-management program is effective in managing fluid overload and should be implemented to promote self-care and reduce complications in patients.
