Abstract

This action research aims to develop a model for accessing stroke services in the Chiang Khan Walking Street community, Chiang Khan District, Loei Province. The sample was randomly selected from individuals who met the inclusion criteria: 1) a multidisciplinary team of 9 members, 2) stakeholders involved in service provision, totaling 56 individuals, and 3) stroke patients, totaling 10 individuals, making a total of 75 participants. Research tools included personal information, focus group discussions, stroke knowledge questionnaires, satisfaction assessments, and quality of care records, which encompassed access to the Stroke Fast Track, transfers within 30 minutes, and the average time from symptom onset to hospital arrival. The reliability of the questionnaires was found to be 0.84 and 0.87. Data were analyzed using content analysis and descriptive statistics.

The research findings revealed that the developed model for accessing stroke treatment services includes symptom monitoring, emergency reporting, referral coordination, and systematic entry into the stroke fast track system. After implementing the model, patients were able to access the Stroke Fast Track 45.65% of the time, with 100% being referred within 30 minutes. The average time from the onset of symptoms to hospital arrival was 2 hours and 40 minutes. The average knowledge score about stroke before and after using the model for accessing stroke services showed a statistically significant difference (p< 0.001), and the satisfaction level with the model was high mean = 8.45, SD =1.68). The research results indicate that the developed model enhances the efficiency of accessing stroke treatment services in tourist communities with contextual limitations. Recommendation: There should be continuous monitoring of the use of the stroke service access model in the community to establish nursing care standards for patients.
