บทคัดย่อ

บทนำ: ผู้ป่วยระยะกลางต้องได้รับการฟื้นฟูสมรรถภาพอย่างต่อเนื่องโดยทีมสหวิชาชีพ แต่การดูแลในปัจจุบันยังขาดความเชื่อมโยงระหว่างโรงพยาบาลและชุมชน

วัตถุประสงค์การวิจัย: เพื่อพัฒนาการมีส่วนร่วมของภาคีเครือข่ายดูแลผู้ป่วยระยะกลางจากโรงพยาบาลสู่ชุมชน

ระเบียบวิธีวิจัย: การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม ดำเนินการระหว่างเดือนมีนาคม - พฤศจิกายน พ.ศ. 2568 ในอำเภอท่าบ่อ จังหวัดหนองคาย ประเทศไทย ผู้ให้ข้อมูลจำนวน 55 คน ประกอบด้วย ผู้ป่วยระยะกลาง 15 คน ผู้ดูแลหรือญาติ 15 คน และผู้ให้บริการทางสุขภาพ 25 คน เครื่องมือที่ใช้ ได้แก่ แบบสัมภาษณ์กึ่งมีโครงสร้าง แนวทางการสังเกตการมีส่วนร่วม แผนการดูแลผู้ป่วยระยะกลาง และแบบประเมิน Barthel ADL Index วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และการวิเคราะห์เชิงเนื้อหา

ผลการวิจัย: การพัฒนาการมีส่วนร่วมประกอบด้วย 5 องค์ประกอบหลัก คือ ระบบคัดกรองและประเมิน แผนการดูแลแบบบูรณาการ การพัฒนาศักยภาพทีม ระบบสารสนเทศ และการมีส่วนร่วมของชุมชน โดยจัดตั้งทีมเครือข่ายที่ประกอบด้วยทีมโรงพยาบาล ทีมโรงพยาบาลส่งเสริมสุขภาพตำบล และทีมชุมชน ผลของการพัฒนาการมีส่วนร่วมของภาคีเครือข่ายพบว่า คะแนนเฉลี่ย Barthel ADL Index ของผู้ป่วยเพิ่มขึ้นจาก 6.8 คะแนน (SD = 2.90) เป็น 11.4 คะแนน (SD = 3.20)

สรุปผล: การพัฒนาการมีส่วนร่วมของภาคีเครือข่ายดูแลผู้ป่วยระยะกลางจากโรงพยาบาลสู่ชุมชน ส่งผลให้ผู้ป่วยมีสมรรถภาพดีขึ้น และเกิดเครือข่ายความร่วมมือที่มีประสิทธิภาพระหว่างโรงพยาบาลและชุมชน

ข้อเสนอแนะ: ควรมีการขยายผลการดำเนินงานไปยังโรงพยาบาลชุมชนอื่น รวมทั้งพัฒนาระบบสารสนเทศที่เชื่อมโยงข้อมูลผู้ป่วยระยะกลางทั้งระบบ และมีการติดตามผลระยะยาวเกิน 6 เดือนเพื่อประเมินความยั่งยืนของการดูแล
