ผลของโปรแกรมส่งเสริมการจัดการตนเองต่อการปรับเปลี่ยนพฤติกรรมสุขภาพ และชะลอภาวะหย่อนสมรรถภาพทางเพศในผู้ป่วยเบาหวานเพศชาย ที่ไม่สามารถควบคุมระดับน้ำตาลในเลือดได้

บทนำ: ภาวะหย่อนสมรรถภาพทางเพศเป็นภาวะแทรกซ้อนสำคัญของโรคเบาหวานเพศชายที่ไม่สามารถควบคุมระดับน้ำตาลในเลือดได้ การพัฒนาโปรแกรมที่เน้นการจัดการตนเองอาจเป็นแนวทางในการปรับเปลี่ยนพฤติกรรมสุขภาพ และชะลอภาวะแทรกซ้อนดังกล่าวได้
วัตถุประสงค์การวิจัย: เพื่อศึกษาผลของโปรแกรมส่งเสริมการจัดการตนเองต่อพฤติกรรมสุขภาพ ระดับน้ำตาลสะสมในเลือด และภาวะหย่อนสมรรถภาพทางเพศในผู้ป่วยเบาหวานเพศชายที่ควบคุมระดับน้ำตาลในเลือดไม่ได้
ระเบียบวิธีวิจัย: การวิจัยกึ่งทดลองแบบสองกลุ่มวัดผลก่อน-หลัง ในผู้ป่วยเบาหวานชนิดที่ 2 เพศชาย อายุ ≥ 35 ปี ระดับ HbA1c ≥ 7 % จำนวน 54 คน คัดเลือกเป็นกลุ่มทดลองและควบคุม กลุ่มละ 27 คน กลุ่มทดลองเข้าร่วมโปรแกรมส่งเสริมการจัดการตนเอง 12 สัปดาห์ กลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือวิจัย ได้แก่ แบบสอบถามข้อมูลส่วนบุคคล และสุขภาพ แบบประเมินพฤติกรรมสุขภาพ และแบบประเมินสมรรถภาพทางเพศ มีค่าความเชื่อมั่นเท่ากับ .80, .99 และ .92 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และสถิติทดสอบแบบนอนพาราเมตริก ได้แก่ การทดสอบวิลคอกซันและการทดสอบแมน–วิทนีย์ยู
ผลการวิจัย: ภายหลังการทดลอง กลุ่มทดลองมีค่ามัธยฐานของพฤติกรรมสุขภาพดีขึ้นอย่างมีนัยสำคัญทางสถิติ (p < .001) ค่ามัธยฐานของระดับ HbA1c ลดลงจาก 8.70 เป็น 7.10 (p < .001) ทั้งสองค่าดีขึ้นมากกว่ากลุ่มควบคุม และค่ามัธยฐานของคะแนนสมรรถภาพทางเพศของกลุ่มทดลองเพิ่มขึ้นจาก 19.00 เป็น 22.00 (p < .001) เมื่อเปรียบเทียบกับกลุ่มควบคุมที่ไม่ได้เปลี่ยนแปลงอย่างมีนัยสำคัญทางสถิติ
สรุปผล: โปรแกรมฯ สามารถใช้ส่งเสริมพฤติกรรมสุขภาพ ควบคุมระดับน้ำตาล และชะลอภาวะหย่อนสมรรถภาพทางเพศในผู้ป่วยเบาหวานเพศชายได้อย่างมีประสิทธิภาพ
ข้อเสนอแนะ: ควรมีการขยายผลโปรแกรมในระดับชุมชนและติดตามผลระยะยาว เพื่อประเมินความยั่งยืนของการเปลี่ยนแปลงพฤติกรรมสุขภาพและผลลัพธ์ทางคลินิก
