บทคัดย่อ

บทนำ: ระยะปากมดลูกเปิดเร็ว เป็นระยะที่ปากมดลูกมีการเปิดขยายอย่างต่อเนื่อง มักก่อให้เกิดความปวดความทุกข์ทรมานแก่ผู้คลอด และเกิดประสบการณ์ด้านลบได้

วัตถุประสงค์การวิจัย: เพื่อพัฒนาแนวปฏิบัติการสนับสนุนในระยะปากมดลูกเปิดเร็ว ณ แผนกห้องคลอด โรงพยาบาลเลย

ระเบียบวิธีวิจัย: การวิจัยและพัฒนานี้ ประยุกต์ใช้กรอบแนวคิดการพัฒนาแนวปฏิบัติของไอโอวา แบ่งเป็น 3 ระยะ ได้แก่ 1) ระยะเตรียมการ ศึกษาสถานการณ์และพัฒนาปฏิบัติ 2) ระยะดำเนินการ นำแนวปฏิบัติไปใช้กับพยาบาลวิชาชีพ 14 ท่าน พนักงานช่วยเหลือคนไข้ 3 ท่าน ผู้คลอด 60 ราย และ 3) ระยะประเมินผล วิเคราะห์และสรุปผลการใช้แนวปฏิบัติฯที่พัฒนาขึ้น เครื่องมือวิจัยประกอบด้วย แนวปฏิบัติการสนับสนุนในระยะปากมดลูกเปิดเร็ว มีค่าความตรงเชิงเนื้อหา(I-CVI = .99, S-CVI = .99) เก็บรวบรวมข้อมูลโดยใช้แบบประเมินการรับรู้ประสบการณ์การคลอด และแบบสอบถามความคิดเห็นของผู้ใช้แนวทางปฏิบัติฯ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณณา และสถิติ Independent t-test

ผลการวิจัย: 1) ด้านผู้คลอด กลุ่มทดลองมีคะแนนการรับรู้ประสบการณ์การคลอดสูงกว่า (M = 138.57) และมีระยะเวลาระยะปากมดลูกเปิดเร็วสั้นกว่า (M = 171.16 นาที) อย่างมีนัยสำคัญทางสถิติ (p < .001) 2) ด้านการนำใช้แนวปฏิบัติ พบว่า พยาบาลและพนักงานช่วยเหลือคนไข้ ปฏิบัติตามแนวทางได้ครบถ้วนร้อยละ 100 และมีความพึงพอใจในระดับสูง (3.77 – 4.80)

สรุปผล: แนวปฏิบัติการสนับสนุนในระยะปากมดลูกเปิดเร็วฯ ที่พัฒนาขึ้น พยาบาลวิชาชีพและผู้ช่วยเหลือคนไข้ สามารถปฏิบัติตามได้ครบถ้วน และมีความพึงพอใจในระดับสูง ผู้คลอดมีประสบการณ์เชิงบวกและมีระยะเวลาระยะปากมดลูกเปิดเร็วสั้นลง

ข้อเสนอแนะ: แนวปฏิบัติฯ สามารถนำไปใช้ในการดูแลผู้คลอดระยะปากมดลูกเปิดเร็ว เพื่อสนับสนุนความก้าวหน้าการคลอด ส่งเสริมประสบการณ์การคลอดที่ดี และลดระยะเวลาการคลอด
